儿童慢性咳嗽的可能性诱发因素研究进展

摘　　要：儿童慢性咳嗽作为一种临床综合征,不仅诱发病因繁杂,而且会累及患儿的多个系统,由于目前临床尚未发现操作简便且具有特异性的检查手段,所以临床确诊较困难。针对慢性咳嗽的定义不同国家存在差异性,例如欧洲呼吸学会在《关于慢性咳嗽的诊断与处理》中指出,无论是儿童还是成年人,咳嗽的持续时间>8周方可定义为慢性咳嗽;美国胸科医师协会、澳大利亚以及新西兰的胸科学会认为咳嗽的持续时间>4周即可定义为慢性咳嗽;而我国中华医学会儿科学分会呼吸学组在《儿童慢性咳嗽诊断与治疗指南》中指出,儿童咳嗽持续时间>4周时,即属于慢性咳嗽。目前临床统计发现,儿童慢性咳嗽的影响因素常见于胃食管反流性疾病、哮喘及相关疾病以及支气管炎等,除此之外,也有一些患儿是因为心理性咳嗽而发展为慢性咳嗽。对此,为确保慢性咳嗽患儿得到妥善治疗,临床医生应准确把握患儿的既往病史,明确其诱发慢性咳嗽的可能性因素,所以本文结合既往研究资料对儿童慢性咳嗽的可能性诱发因素进行综述,希望可以为相关人员提供参考。As a clinical syndrome,chronic cough in children not only induces complicated etiologies,but also affects multiple systems of the child.Since no simple and specific examination methods have been found in clinical practice,clinical diagnosis is difficult.There are differences in the definition of chronic cough in different countries.For example,the European Respiratory Society points out in the Diagnosis and Management of Chronic Cough that whether it is a child or an adult,a cough period of more than 8 weeks can be defined as a chronic cough;American Thoracic Association of Physicians,the Thoracic Society of Australia and New Zealand believe that a cough period of more than 4 weeks can be defined as a chronic cough;while the Respiratory Group of the Pediatrics Branch of the Chinese Medical Association points out in the Guidelines for the Diagnosis and Treatment of Chronic Cough in Children,when children cough for more than 4 weeks,it is a chronic cough.Current clinical statistics have found that the influencing factors of chronic cough in children are commonly found in gastroesophageal reflux disease,asthma and related diseases,and bronchitis.In addition,some children develop chronic cough due to psychological cough.In this regard,in order to ensure that children with chronic cough are properly treated,clinicians should accurately grasp the children’s past medical history and clarify the possible factors that induce chronic cough.Therefore,combined with previous research data,this article gives an overview of the possible inducing factors of chronic cough in children,hoping to provide reference for relevant personnel.
