摘 要:目的 研究隐匿性高血压(MH)患者内皮依赖性血管舒张功能情况。方法 选取2019年6月—2021年6月于东莞市人民医院门诊及住院确诊为隐匿性高血压患者40例纳入观察组,将同期就诊的40例持续性高血压患者(SH)纳入对照组A组,并选取同期健康检查结果正常者40例作为对照组B组。三组患者均以超声进行肱动脉血流介导的血管扩张功能(FMD)及颈动脉内膜中层厚度(IMT)监测,对比不同组别患者的FMD、IMT差异。结果 观察组及对照组A组患者FMD指标对比差异无统计学意义(P>0.05);两组FMD指标均较对照组B组低,差异有统计学意义(P<0.05)。观察组IMT高于对照组A组,两组IMT均高于对照组B组,差异均有统计学意义(P<0.05)。结论 MH患者及SH患者较正常人内皮依赖性血管舒张功能受到损害,MH患者及SH患者两者之间无明显差异。对MH患者的内皮依赖性血管舒张功能指标FMD及IMT进行分析,可以评估高血压血管损害进展及其危害程度,可以为以后进一步研究MH对靶器官的损害机制探索方向及提供研究基础。Objective To study the endothelium-dependent vasodilation in patients with masked hypertension(MH). Methods A total of 40 outpatients and inpatients with occult hypertension diagnosed in Dongguan People’s Hospital from June 2019 to June 2021 were included in the observation group,40patients with sustained hypertension(SH)in the same period were included in the control group A,and 40people with normal health examination results in the same period were selected as the control group B.Theflow mediated vasodilation function(FMD)of brachial artery and intima-media thickness(IMT)of carotid artery were monitored by ultrasound in the three groups. The differences of FMD and IMT in different groups were compared. Results There was no significant difference in FMD indexes between observation group and control group A(P>0.05);FMD indexes of the two groups were lower than those of control group B,and the differences were statistically significant(P<0.05). The IMT of the observation group was higher than that of the control group A,and the IMT indexes of the two groups were higher than those of group B in the control group,and the differences were statistically significant(P<0.05). Conclusion The endotheliumdependent vasodilation function of MH patients and SH patients were impaired compared with normal people,and there was no significant difference between MH patients and SH patients. The analysis of endothelial dependent vasodilation function indexes FMD and IMT in patients with MH could evaluate the progress and harm degree of vascular damage in hypertension,and provide a research basis for further studying the mechanism of MH damage to target organs in the future.
