奥硝唑片联合黄连上清片致重症多形红斑1例及文献分析

摘　　要：1例67岁男性患者,2020年10月25日因牙痛自行服药奥硝唑片、黄连上清片,服药后第2天躯干四肢出现红斑(直径1~3 cm不等)。皮肤症状呈进行性加重。2020年10月31日入院治疗。皮肤科会诊,诊断为重症多形红斑,给予糖皮质激素、口服抗组胺药、补液等对症支持治疗,皮肤症状好转。入院第9天出现呼吸道症状,给予对症治疗,2020年10月28日好转出院。本文采用Naranjo评分法,最终评定结果提示:奥硝唑片5分,评定结果很可能;黄连上清片4分,评定结果可能。不排除患者联合用药导致严重不良反应可能,临床应引以为戒,对有过敏史患者应注意密切监测,尽量避免联合用药。A 67-year male patient took Ornidazole Tablets and Huanglian Shangqing Tablets due to toothache on October 25,2020.The trunk and limbs appeared erythema on the next day of oral administration,and the skin symptoms were progressively ag⁃gravated.On October 31,2020,the patient was admitted to the hospital.Upon the dermatology department consultation,the patient was diagnosed as severe erythema multiforme.Symptomatic and supportive treatments such as glucocorticoids,oral antihistamines,and rehydration were given,and the skin symptoms improved.Respiratory symptoms appeared on the 9th day of admission,and symptomatic treatment was given.On October 28,2020,the patient improved and was discharged.The Naranjo scoring method was used in this article,and the final evaluation result suggested that:Ornidazole Tablets 5 points,the evaluation result was very pos⁃sible;Huanglian Shangqing Tablets 4 points,the evaluation result was possible.The possibility of serious adverse reactions caused by combined medication was not ruled out,and clinical caution should be taken as a warning.Patients with a history of allergies should be closely monitored,and combination therapy should be avoided as much as possible.
