两种幽门螺杆菌检测方法的临床比较

摘　　要：目的:分析胃黏膜组织的聚合酶链反应(qPCR)检测与胃液快速尿素酶试验在幽门螺杆菌(HP)检测中的应用价值。方法:选取2020年5-9月因上腹部不适在江门市中心医院进行胃镜检查的患者92例,分别予以胃黏膜组织的qPCR检测、胃液快速尿素酶试验、胃黏膜组织病理检测三种方法进行检测幽门螺杆菌。比较三种检测方法的HP检出情况。结果:以病理学结果为金标准,尿素酶试验的阳性率高于病理学检查和qPCR,差异有统计学意义(P<0.05)。qPCR与尿素酶试验的敏感性比较,差异无统计学意义(P>0.05);qPCR的特异性高于尿素酶试验,差异有统计学意义(P<0.05)。结论:qPCR和尿素酶试验的敏感性相当,均能有效检出HP感染,而qPCR特异性高于尿素酶试验,且不受药物影响,保证了胃肠道疾病中抗生素及抑酸剂等药物的应用。Objective:To analyze the application value of polymerase chain reaction(qPCR)detection of gastric mucosal tissue and rapid urease test of gastric juice in the detection of Helicobacter pylori(HP).Methods:From May 2020 to September 2020,92 patients who underwent gastroscopy in Jiangmen Central Hospital due to upper abdomen discomfort were selected.They were detected by three methods:qPCR of gastric mucosa,rapid urease test of gastric juice and pathological examination of gastric mucosa to detect HP.The HP detection status of the three detection methods was compared.Results:Taking pathological results as the gold standard,the positive rate of urease test was significantly higher than that of pathological examination and qPCR,and the difference was statistically significant(P<0.05).There was no significant difference in the sensitivity between qPCR and urease test(P>0.05).The specificity of qPCR was significantly higher than that of urease test,and the difference was statistically significant(P<0.05).Conclusion:The sensitivity of qPCR and urease test is equivalent,and both methods can effectively detect HP infection.The specificity of qPCR is higher than that of urease test,and it is not affected by drugs,ensuring the application of antibiotics and acid inhibitors in gastrointestinal diseases.
