摘　　要：目的探讨载脂蛋白E(ApoE)基因表型与糖尿病患者并发心血管疾病的相关性,协助风险预估,指导临床合理用药。方法选取上海市杨浦区市东医院2020年1月—2020年10月糖尿病患者共155例,采用基因芯片法对其ApoE基因进行分型检测,比较不同基因型与糖尿病患者发生心血管疾病的关系。结果155例糖尿病患者中E2/E21例(0.65%)E2/E317例(10.97%),E2/E41例(0.65%),E3/E3109例(70.32%),E3/E425例(16.13%),E4/E42例(1.29%),E3/E3基因型占比最高。糖尿病患者E3基因型(E2/E4、E3/E3)和E4基因型(E3/E4、E4/E4)发生动脉粥样硬化概率高达68%以上,E2基因型(E2/E2、E2/E3)在糖尿病患者合并动脉粥样硬化的比例为22%;糖尿病合并冠心病的占比分别是E2基因型4.5%、E3基因型30.2%、E4基因型32.25%。3个基因型与糖尿病患者外周血管硬化、冠心病发生具有统计学意义(P<0.05)。结论糖尿病患者ApoE基因型以E3型为主,糖尿病患者发生心血管疾病的风险从高到低依次为E4、E3、E2型。临床可根据ApoE不同基因型制定出适合患者的个体化治疗方案。Objective To investigate the correlation between apolipoprotein E(ApoE)gene phenotype and cardiovascular disease in diabetic patients,and to assist in risk prediction and guide clinical rational drug use.Methods A total of 155 patients with diabetes mellitus were selected from January 2020 to October 2020.ApoE gene typing was detected by gene chip method,and the relationship between different genotypes and the occurrence of cardiovascular disease in diabetic patients was compared.Results There were 1 case of with E2/E2(0.65%),17 cases of with E2/E3(10.97%),1 case of withE2/E4(0.65%),109 cases of with E3/E3(70.32%),25 cases of with E3/E4(16.13%),and 2 cases of with E4/E4(1.29%)in 155 diabetic patients.E3/E3 genotype accounted for the highest proportion.The incidence of atherosclerosis in diabetic patients with E3 genotype(E2/E4,E3/E3)and E4 genotype(E3/E4,E4/E4)was more than 68%,and the incidence of atherosclerosis in diabetic patients with E2 genotype(E2/E2,E2/E3)was 22%.The proportion of diabetic patients with coronary heart disease was 4.5%in E2 genotype,30.2%in E3 genotype and 32.25%in E4 genotype.The three genotypes had statistical significance with the occurrence of peripheral vascular sclerosis and coronary heart disease in diabetic patients(P<0.05).Conclusion ApoE genotype in diabetic patients is mainly E3 type,and the risk of cardiovascular disease in diabetic patients is E4,E3 and E2 type from high to low.Clinical treatment can be tailored according to different ApoE genotypes.
