摘  要:目的探究亚低温联合重组人促红细胞生成素对重度缺氧缺血性脑病新生儿(HIE)临床疗效及行为神经评分的影响。方法选取2018年6月—2020年8月濮阳市人民医院接受的68例重度HIE新生儿作为研究对象,以随机数字表法分为参照组(n=34)和研究组(n=34),两组患儿均行常规治疗,其中参照组患儿在此基础上使用亚低温治疗,研究组患儿在参照组基础上使用重组人促红细胞生成素(rh-EPO)治疗,比较两组患儿神经功能评分,智能发育情况及临床效果。结果治疗1周、2周后两组新生儿神经行为测定(NBNA)评分均升高,且研究组高于参照组,随着时间延长,评分逐渐升高(P<0.05);治疗后6个月,两组患儿精神运动发育指数(PDI)、智力发育指数(MDI)及精神运动发育商(MDQ)评分均升高,且研究组高于参照组(P<0.05);研究组总有效率高于参照组(P<0.05)。结论亚低温联合rh-EPO治疗重度HIE患儿可显著提高临床疗效,降低患儿的神经功能损伤,促进患儿智力及精神发育,效果显著。Objective To explore the effect of mild hypothermia combined with recombinant human erythropoietin on the clinical efficacy and behavioral neurological score of newborns with severe hypoxic-ischemic encephalopathy(HIE).Methods Totally 68 cases of severe HIE newborns accepted by our hospital from June 2018 to August 2020 were selected as the research objects,and they were divided into the reference group(n=34)and the study group(n=34)by the random number table method.Children in the reference group were treated with mild hypothermia on this basis,and the children in the study group were treated with recombinant human erythropoietin(rh-EPO)on the basis of the reference group.The neurological functions,intelligence development and clinical effect of the two groups were compared.Results After 1 week and 2 weeks of treatment,the neonatal neurobehavioral measurement(NBNA)scores of the two groups increased,and the study group was higher than that of the reference group,and the scores gradually increased with time(P<0.05).6 months after treatment psychomotor development index(PDI),mental development index(MDI)and psychomotor development quotient(MDQ)scores of the two groups of children increased,and the study group were higher than those the reference group(P<0.05),the total effective rate of the study group was higher than that of the reference group(P<0.05).Conclusion Mild hypothermia combined with rh-EPO in the treatment of children with severe HIE can significantly improve the clinical efficacy,reduce the children’s neurological damage,and promote the children’s intellectual and spiritual development,with significant effects.
