许家骏从“风、热、瘀”论治巨乳头性结膜炎的临证经验

摘　　要：近年来,巨乳头性结膜炎发病率逐渐升高,此病属于中医学“目痒症”“时复症”范畴。许家骏认为巨乳头性结膜炎的核心病因病机是风、热、瘀三者并见,病机演变从表入里,风、热、瘀交结,重点在瘀。根据患者临床特征,可将其主要分为风热夹瘀型、瘀热互结型和气滞血瘀型。治以疏风清热、凉血活血、行气散瘀,辨证分型疗效显著。分期论治推荐治疗初期以银翘散疏风清热,中期以升降散升清降浊,后期以桃红四物汤活血化瘀,充分体现了中医辨证论治,方随证转的特色与优势,为巨乳头性结膜炎的治疗提供新的启发。In recent years,the incidence rate of giant papillary conjunctivitis increases gradually,and this disease belongs to the category of"itchy eyes"and"seasonal eye disease"of Traditional Chinese Medicine(TCM).XU Jiajun believes that the core etiology and pathogenesis of giant conjunctivitis are the combination of blood stasis,wind and heat.The pathogenesis enters from the exterior to the interior,wind intertwined with heat and blood stasis and blood stasis is the key role.According to the clinical characteristics,they can be mainly divided into wind heat with blood stasis type,binding of blood stasis and heat type as well as blood stasis due to Qi stagnation type.It is treated by dispersing wind and clearing heat,cooling blood and activating blood circulation,promoting Qi and dispersing blood stasis.The effect of the treatment based on syndrome differentiation is remarkable.It is recommended to treat by stages of disease.Specifically,Yinqiao Powder is used to dissipate wind and clear heat in the initial stage.Shengjiang Powder is used to upbear clear Qi and downbear turbid Qi in the middle stage.Taohong Siwu Decotion is used to activate blood circulation and remove blood stasis in the later stage.It fully embodies the characteristics and advantages of syndrome differentiation and treatment as well as syndrome-based prescription of TCM.The thought of XU Jiajun can provide new inspiration for the treatment of giant papillary conjunctivitis.
