515例单脐动脉胎儿合并单一畸形及多发畸形情况分析

摘　　要：目的探讨单脐动脉(SUA)胎儿合并的畸形的发生率及合并单一畸形、多发畸形情况分析。方法回顾性分析2014—2019年在嘉兴市妇幼保健院诊断为单脐动脉孕产妇的病例共515例(均为单胎妊娠),就胎儿合并的畸形的发生率及合并单一畸形、多发畸形情况分析。结果515例单脐动脉胎儿中,合并胎儿畸形的有165例占32.04%,其中心血管畸形60例占36.36%、中枢神经系统畸形50例占30.30%、泌尿生殖系统畸形48例占29.09%、骨关节畸形47例占28.48%、颜面部畸形25例占15.15%、消化系统畸形20例占12.12%、腹壁畸形17例占10.30%、羊水异常17例占10.30%、胎盘早剥1例占0.61%、死胎1例占0.61%。结论单脐动脉易伴发其他先天畸形,妊娠期间发现单脐动脉胎儿应及时系统的对其主要器官、系统进行畸形筛查,及早发现畸形及时予以干预,妊娠期及分娩过程中应加强胎儿监护。Objective To explore the occurrence of single malformation and multiple malformations in 515 fetuses with single umbilical artery(SUA).Methods From 2014 to 2019,515 cases(single pregnancy)of SUA in Jiaxing Municipal Maternal and Child Health Care Hospital were analyzed retrospectively,the incidence rate of fetal malformations and the occurrence of single malformation and multiple malformations were analyzed.Results Among 515 SUA cases,165 cases were found with fetal malformations,accounting for 32.04%,including 60 cases of cardiovascular malformations(36.36%),50 cases of central nervous system malformations(30.30%),48 cases of genitourinary system malformations(29.09%),47 cases of osteoarticular malformations(28.48%),25 cases of facial malformations(15.15%),20 cases of digestive malformations(12.12%),17 cases of abdominal malformations(10.30%),17 cases of abnormal amniotic fluid(10.30%),1 case of placental abruption(0.61%),and 1 case of stillbirth(0.61%).Conclusion SUA appears to be associated with other congenital malformations,major organ and system malformation screening should be conducted in SUA cases during pregnancy,early detection and timely intervention are necessary,fetal monitoring should be enhanced during pregnancy and labor.
