基于MR增强图像全瘤直方图分析鉴别诊断腮腺Warthin瘤与多形性腺瘤

摘　　要：目的:探讨基于腮腺MR增强图像全瘤直方图分析鉴别诊断Warthin瘤与多形性腺瘤的价值。方法:回顾性分析经临床确诊的16例Warthin瘤与8例多形性腺瘤患者治疗前MR资料。所有患者均在GE1.5T下行腮腺MR平扫及增强扫描,利用Omni-Kinetics软件后处理得到肿瘤区域增强纹理参数,进行组间比较,并利用受试者操作特性(ROC)曲线评价各参数的鉴别诊断效能。结果:MR增强图像直方图参数中Warthin瘤的偏度、峰度、能量、熵、正像素一致性分别为0.678±0.751、2.234±1.478、0.012±0.002、6.723±0.215、0.012±0.002,多形性腺瘤各值分别为-0.340±0.671、0.618±0.883、0.009±0.003、7.075±0.353、0.009±0.003,组间比较差异均具有统计学意义(P<0.05);ROC曲线的AUC分别为0.836、0.844、0.820、0.211、0.820,其中以峰度诊断效能较高。结论:基于腮腺MR增强图像全瘤直方图分析参数可为Warthin瘤与多形性腺瘤的鉴别诊断有较高价值。Objective:To explore the value of differential diagnosis of Warthin tumor and pleomorphic adenoma based on whole tumor histogram analysis of enhanced MR image of parotid gland.Methods:The MR data of 16 patients with Warthin tumor and 8 patients with pleomorphic adenoma before treatment were retrospectively analyzed.All patients underwent MR and contrast-enhanced scanning of the parotid gland under GE1.5 T.Omni-Kinetics software was used for post-processing to obtain enhanced texture parameters of the tumor region,and the comparison between groups was made.Receiver-operating characteristic(ROC)curves were used to evaluate the efficacy of each parameter in differential diagnosis.Results:The skewness,kurtosis,energy,entropy and positive pixel consistency of Warthin tumor in the histogram parameters of MR enhanced image were 0.678±0.751,2.234±1.478,0.012±0.002,6.723±0.215,0.012±0.002,respectively.The values of pleomorphic adenoma were-0.340±0.671,0.618±0.883,0.009±0.003,7.075±0.353 and 0.009±0.003,respectively,and the differences among groups were statistically significant(P<0.05).The AUC of ROC curve was 0.836,0.844,0.820,0.211 and 0.820,respectively,among which the diagnostic efficiency of kurtosis was higher.Conclusion:The histogram analysis parameters based on the enhanced MR image of the parotid gland can be used for the differential diagnosis of Warthin tumor and pleomorphic adenoma.
