摘　　要：目的探讨痰热清注射液联合支气管镜下肺泡灌洗治疗重症肺炎的效果及对炎性因子、晚期糖化终产物受体(RAGE)、可溶性髓样细胞触发受体-1(sTREM-1)水平的影响。方法选择2019年6月至2020年10月我院收治的92例重症肺炎患者作为研究对象,根据治疗方案将其分为对照组(46例,支气管镜下肺泡灌洗)和观察组(46例,痰热清注射液+支气管镜下肺泡灌洗)。比较两组患者的临床症状消失时间、炎性因子、RAGE、sTREM-1水平及生活质量。结果观察组的咳嗽、咳痰、胸痛、肺部湿啰音消失时间及退热时间均短于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的干扰素-γ(IFN-γ)、降钙素原(PCT)、尿白三烯E4(LTE4)水平均降低,且观察组明显低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的RAGE、sTREM-1水平均降低,且观察组明显低于对照组,差异具有统计学意义(P<0.05)。治疗后,观察组的世界卫生组织生活质量量表(WHOQOL-BREF)各维度评分均升高,且观察组明显高于对照组,差异具有统计学意义(P<0.05)。结论痰热清注射液联合支气管镜下肺泡灌洗治疗重症肺炎的效果显著,可缩短各项临床症状消失时间,调节机体炎性因子、RAGE、sTREM-1水平,进而提高患者的生活质量,值得临床推广应用。Objective To explore the effect of Tanreqing injection combined with bronchoscopic alveolar lavage in the treatment of severe pneumonia and its influences on the levels of inflammatory factors,receptor for advanced glycation end-products(RAGE)and soluble triggering receptor expressed on myeloid cells-1(sTREM-1).Methods A total of 92 patients with severe pneumonia who were admitted in our hospital from June 2019 to October 2020 were selected as the research objects,and the patients were divided into control group(46 cases,bronchoscopic alveolar lavage)and observation group(46 cases,Tanreqing injection+bronchoscopic alveolar lavage)according to the treatment plan.The clinical symptoms disappeared time,inflammatory factors,RAGE,sTREM-1 levels and quality of life were compared between the two groups.Results The disappearance time of cough,expectoration,chest pain,pulmonary moist rales and antipyretic time in the observation group were shorter than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of interferon-γ(IFN-γ),procalcitonin(PCT)and leukotriene E4(LTE4)in the two groups decreased,those in the observation group were significantly lower than the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of RAGE and sTREM-1 in both groups decreased,those in the observation group were significantly lower than the control group,and the differences were statistically significant(P<0.05).After treatment,the scores of each dimension of the World Health Organization Quality of Life-BREF(WHOQOL-BREF)in the observation group increased,those in the observation group were significantly higher than the control group,and the differences were statistically significant(P<0.05).Conclusion Tanreqing injection combined with bronchoscopic alveolar lavage in the treatment of severe pneumonia has a significant effect,it can shorten the disappearance time of various clinical symptoms,regulate the levels of inflammatory factors,R
