《金匮要略》黄芪桂枝五物汤治疗糖尿病周围神经病变气虚血瘀证临床观察

摘　　要：目的观察《金匮要略》黄芪桂枝五物汤原方配伍比例下治疗糖尿病周围神经病变(DPN)气虚血瘀证疗效。方法130例DPN患者随机分为观察组(黄芪桂枝五物汤)和对照组(甲钴胺片),观察干预12周前后糖代谢指标[空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)],多伦多临床神经病变量表(TCSS),中医证候,腓总神经和正中神经的运动神经传速度(MNVC)及感觉神经传导速度(SNVC),比较2组临床疗效及安全性。结果与对照组治疗后比较,观察组TCSS、中医证候积分、FPG、2 h PG、HbA1c均降低(P<0.05);腓总神经和正中神经MNVC、SNVC增快(P<0.05),总有效率高、不良反应低(P<0.05)。结论《金匮要略》黄芪桂枝五物汤原方配伍比例下治疗糖尿病周围神经病变疗效佳。Objective To observe the curative effect of Huangqi Guizhi Wuwu decoction in Golden Chamber Synopsis in the treatment of diabetic peripheral neuropathy with qi deficiency and blood stasis syndrome.Methods 130 patients with diabetic peripheral neuropathy were randomly divided into observation group(Huangqi Guizhi Wuwu decoction)and control group(mecobalamin tablet).Glucose metabolism indicators before and after the intervention for 12 weeks,TCSS,traditional Chinese medicine syndrome score,and General Filipina Nerve and median nerve MNVC and SNVC were observed,and the clinical efficacy and safety of the two groups was comapred.Results Compared with the control group after treatment,the observation group’s TCSS,traditional Chinese medicine syndromes,FPG,2 h PG,and HbA1 all decreased(P<0.05);the common peroneal nerve and median nerve MNVC and SNVC increased faster(P<0.05),the total effective rate was high,and the adverse reactions were low(P<0.05).Conclusion Huangqi Guizhi Wuwu decoction in Golden Chamber Synopsis has a good effect in treating diabetic peripheral neuropathy with qi deficiency and blood stasis syndrome.
