摘  要:目的分析支气管哮喘合并支原体感染患者白细胞介素1受体样分子1(IL1RL1)、白细胞介素33(IL-33)与哮喘发作的相关性。方法收集2018年1月至2020年12月铜川矿务局中心医院收治的104例急性发作期支气管哮喘患者作为急性发作组,并将经性别、年龄成组匹配的104例缓解期支气管哮喘患者作为缓解组,比较两组患者的肺炎支原体(MP)感染阳性率,根据MP感染情况将急性发作组分为急性发作MP阳性组与急性发作MP阴性组,比较三组患者入院次日清晨呼吸道分泌物IL-33、IL1RL1水平及血清IL-33、IL1RL1、免疫球蛋白E(IgE)、外周血嗜酸性粒细胞计数(EOS)的差异,并采用Spearman秩相关分析评估急性发作MP阳性组患者病情严重程度与呼吸道分泌物IL-33、IL1RL1及血清IL-33、IL1RL1、IgE、EOS的相关性。结果急性发作组患者的MP感染阳性率为51.92%,明显高于缓解组的21.15%,差异有统计学意义(P<0.05);急性发作MP阳性组患者的呼吸道分泌物IL-33、IL1RL1及血清IL-33、IL1RL1、IgE、EOS水平明显高于急性发作MP阴性组及缓解组,而急性发作MP阴性组患者的上述各项指标明显高于缓解组,差异均有统计学意义(P<0.05);经Spearman秩相关分析结果显示,病情严重程度与呼吸道分泌物IL-33、IL1RL1及血清IL-33、IL1RL1、IgE、EOS水平呈显著正相关(r=0.655、0.408、0.478、0.563、0.780、0.391,P<0.05)。结论MP感染可能是引起支气管哮喘急性发作的重要原因,其作用机制可能与IL-33/IL1RL1信号通路激活有关。Objective To analyze the correlation between interleukin 1 receptor-like molecule 1(IL1RL1),interleukin 33(IL-33)and asthma attacks in patients with bronchial asthma complicated mycoplasma infection.Methods A total of 104 patients with acute attack of bronchial asthma treated in the Central Hospital of Tongchuan Mining Bureau between January 2018 and December 2020 were enrolled as acute attack group,and another 104 patients with bronchial asthma during remission matched by gender and age were selected as remission group.The positive rate of Mycoplasma pneumoniae(MP)infection was recorded in the two groups of patients.According to the status of MP infection,the patients in the acute attack group were divided into MP-positive acute attack group and MP-negative acute attack group,and the levels of respiratory secretion IL-33 and IL1RL1 and serum IL-33,IL1RL1 and immunoglobulin E(IgE)and peripheral blood eosinophil count(EOS)were compared among the three groups in the next morning after admission.The correlation between severity of disease and respiratory secretion IL-33 and IL1RL1 and serum IL-33,IL1RL1,IgE and EOS among patients in MP-positive acute attack group was evaluated by Spearman rank correlation analysis.Results The positive rate of MP infection of patients in acute attack group was 51.92%,which was significantly higher than 21.15%in remission group(P<0.05).The levels of respiratory secretion IL-33 and IL1RL1 and serum IL-33,IL1RL1,IgE,and EOS in MP-positive acute attack group were significantly higher than those in MP-negative acute attack group and those in remission group(P<0.05),and the levels in MP-negative acute attack group were higher compared with those in remission group(P<0.05).Spearman rank correlation analysis results showed that the severity of disease was significantly positively correlated with respiratory secretion IL-33 and IL1RL1 and serum IL-33,IL1RL1,IgE,EOS(r=0.655,0.408,0.478,0.563,0.780,0.391,P<0.05).Conclusion MP infection may be an important cause of acute attack of bronchial asth
