摘  要:目的探讨“海鸥”皮瓣治疗小儿手部烧伤后指蹼瘢痕粘连的临床效果。方法选择2019年1月至2021年11月北京积水潭医院烧伤科收治的手部烧伤后指蹼瘢痕挛缩患儿19例,年龄2~12岁,共36个烧伤后瘢痕粘连指蹼,均采用“海鸥”皮瓣修复。观察皮瓣成活情况,指蹼外观满意度,相邻两指外展度是否同健侧;采用改良Withey评分对指蹼进行评价,并统计随访期间的再次手术情况。结果所有皮瓣均成活,指蹼外观满意率为100%,相邻两指外展度与健侧相似;Withey评分显示指蹼外观均较术前明显改善(P<0.05);随访3~24个月,所有患儿均未接受再次手术。结论“海鸥”皮瓣血运可靠,易于操作,且修复后外观良好,在小儿指蹼烧伤后瘢痕粘连的治疗中具有较好的应用前景。Objective To investigate the clinical effects of“seagull”skin flap on the treatment of finger web scar adhesion after burn in children.Method We selected 19 burn patients with 36 webbed fingers from Beijing Jishuitan Hospital from January 2019 to November 2021,aged from 2 to 12 years.These fingers were repaired with“seagull”flap.The survival of the flap and the satisfaction of the appearance were evaluated postoperatively.The abductions of the adjacent fingers on the affected side and the opposite side were compared.A modified Withey score was used to evaluate webbed fingers.The secondary operations during the follow-up were analyzed.Result All flaps survived.The patients and their parents were 100%satisfied with the appearance of the webbed fingers.The abduction degree of the two adjacent fingers was similar to that of the opposite side.Withey score showed that the appearance was significantly improved(P<0.05).No reoperation was performed during follow-up period(3-24 months).Conclusion The“seagull”flap is simple to operate and shows good appearance after repair.It has therapeutic prospect in the treatment of scar adhesion after webbed burn in children.
