胸腔置管引流与尿激酶联合治疗结核性渗出性胸膜炎的效果观察

摘　　要：目的观察胸腔置管引流与尿激酶联合治疗结核性渗出性胸膜炎效果分析。方法将在我院(2019年1月—2020年10月时期)收治结核性渗出性胸膜炎为诊断的患者总计206例,根据治疗方案的区别分组,以常规治疗配合胸腔置管引流的98例患者为对照组,而以胸腔置管引流与尿激酶联合治疗的另108例患者为研究组。比较两组患者治疗前、后的血清炎性指标水平,治疗后临床相关指标,并判定治疗效果。结果治疗后两组血清炎性指标水平与治疗前相比下降,且研究组水平更低,差异有统计学意义(P<0.05)。研究组治疗后无胸腔积液、无胸闷气喘的时间与对照组相比更短,抽液总量与对照组相比更多,且胸膜厚度与对照组相比更薄,差异有统计学意义(P<0.05)。研究组患者在治疗后的总有效率与对照组相比更高,临床治疗效果更好,差异有统计学意义(P<0.05)。结论在结核性渗出性胸膜炎患者的治疗中,以胸腔置管引流与尿激酶联合治疗,可以明显的降低血清炎性指标水平,并缩短无胸腔积液、无胸闷气喘的时间,抽液总量更多,胸膜厚度更薄,提升治疗效果。Objective To observe and analyze the clinical effect of pleural drainage combined with urokinase in the treatment of tuberculous exudative pleurisy.Methods The patients with tuberculous exudative pleurisy in our hospital(from January 2019 to October 2020)were divided into two groups according to the different treatment schemes.98 patients with conventional treatment combined with thoracic catheter drainage were selected as the control group,while the other 108 patients with thoracic catheter drainage combined with urokinase were selected as the study group.The levels of serum inflammatory indexes before and after treatment,clinical related indexes after treatment were compared between the two groups,and the therapeutic effect was determined.Results After treatment,the levels of serum inflammatory indexes in the two groups decreased significantly compared with those before treatment,and the levels of serum inflammatory indexes in the study group were lower than those in the control group,with significant difference(P<0.05).Compared with the control group,the time of no pleural effusion,no chest tightness and asthma in the study group was shorter,the total amount of fluid pumping was more,and the pleural thickness was thinner,the difference was significant(P<0.05).The total effective rate of the study group after treatment was higher than that of the control group,the clinical treatment effect was better,the difference was significant(P<0.05).Conclusion In the treatment of patients with tuberculous exudative pleurisy,pleural drainage combined with urokinase can significantly reduce the level of serum inflammatory indexes,shorten the time of no pleural effusion,no chest tightness and asthma,increase the total amount of fluid pumping,thinner pleural thickness,and improve the treatment effect.
