摘  要:目的:研究智能可穿戴十二导联心电图在心血管疾病远程诊疗中的临床应用价值。方法:依托中国胸痛中心“全国心电一张网”平台,选取2019年4月至2020年4月期间使用智能可穿戴十二导联心电图采集并成功上传至心电云平台的使用者心电图资料,对使用者的症状、心电图诊断情况、心电危急值检出情况、疾病转归情况进行分析。结果:研究共纳入66 735例使用者的有效心电图资料,其平均年龄(55.8±16.2)岁,男性36 961例(55.38%),女性29 774例(44.62%)。心电图未见明显异常11 686例(17.51%),窦性心动过缓7 907例(11.85%),窦性心动过速3 102例(4.65%),房性早搏2 092例(3.13%),室性早搏2 631例(3.94%)。ST段/ST-T改变11 558例(17.32%),急性心肌缺血101例(0.15%),陈旧性心肌梗死16例(0.02%),急性心肌梗死12例(0.02%)。共检出危急值心电图340例(0.51%),死亡8例(0.01%),29例(0.04%)无法取得联系,预警后250例(0.37%)去医院就诊,其中155例(0.23%)就诊后接受药物治疗,57例(0.09%)接受手术治疗。结论:智能可穿戴十二导联心电图可远程监测心电状态,从而进行早期诊断和风险预测,有利于使用者及时接受针对心血管急症的医疗干预,减少心血管事件的发生,因此具有较高的应用价值。Objectives: To explore the clinical application value of the intelligent wearable 12-lead electrocardiogram in the remote diagnosis and treatment of cardiovascular diseases.Methods: Relying on China Chest Pain Center "national ECG network" platform, the electrocardiogram data successfully uploaded to the ECG cloud platform by smart wearable 12-lead electrocardiogram from April 2019 to April 2020 were analyzed. The symptoms of patients, ECG diagnosis, critical ECG value detection status and outcome of patients were evaluated.Results: A total of 66 735 effective ECG data were included in the study. The mean age was(55.8±16.2) years, 36 961(55.38%) cases were male. There were 11 686(17.51%) cases with no obvious abnormal ECG. There were 7 907(11.85%)sinus bradycardia cases, 3 102(4.65%) sinus tachycardia cases, 2 092(3.13%) atrial premature beats cases, and 2 631(3.94%)ventricular premature beats cases. There were 11 558(17.32%) cases of ST-segment/ST-T changes, 101(0.15%) cases of acute myocardial ischemia, 16(0.02%) cases of old myocardial infarction, and 12(0.02%) cases of acute myocardial infarction.There were 340(0.51%) patients with critical ECG values, 8(0.01%) died and 29(0.04%) were unable to be contacted. After the warning, 250(0.37%) patients went to the hospital, 155(0.23%) of whom received medical treatment and 57(0.09%)underwent surgery.Conclusions: Intelligent wearable 12-lead electrocardiogram can be used for remote monitoring of ECG status,early diagnosis, and risk stratification, which is valuable for decision making in terms of timely medical intervention in cardiovascular emergencies and reduction in the incidence of cardiovascular events.
