射血分数降低的心力衰竭合并肺部感染患者免疫指标及心肌酶变化的研究

摘　　要：目的 探讨射血分数降低的心力衰竭(HFrEF)合并肺部感染患者免疫指标及心肌酶的变化。方法 回顾性分析2019年3月至2021年4月我院收治的100例HFrEF患者的临床资料,根据是否合并肺部感染将其分为感染组、未感染组,比较两组的免疫指标及心肌酶指标,分析其在HFrEF合并肺部感染患者中的变化。结果 感染组的CD4^(+)/CD8^(+)、 IgG、 IgM水平均低于未感染组(P <0.05)。感染组的CK、 CKMB、 LDH水平均高于未感染组(P <0.05)。CD4^(+)/CD8^(+)、 IgG、 IgM评估HFrEF患者并发肺部感染的AUC为0.804、 0.828、 0.772。结论 HFrEF合并肺部感染患者免疫功能低下、心肌酶升高,检测免疫指标有助于HFrEF合并肺部感染的早期诊断。Objective To explore the changes of immune indicators and myocardial enzymes in patients with heart failure reduced ejection fraction(HFr EF) complicated with pulmonary infection.Methods The clinical data of 100 HFr EF patients admitted to our hospital from March 2019 to April 2021 were analyzed retrospectively,and divided into infected group and uninfected group according to the pulmonary infection.The immune indicators and myocardial enzyme indicators of the two groups were compared,and the changes in patients with HFr EF complicated with pulmonary infection were analyzed.Results The CD4^(+)/CD8^(+),Ig G and Ig M levels of the infected group were lower than those of the uninfected group(P <0.05).The CK,CKMB and LDH levels of the infected group were higher than those of the uninfected group(P <0.05).The AUC of CD4^(+)/CD8^(+),Ig G and Ig M in the evaluation of HFr EF patients with pulmonary infection were0.804,0.828 and 0.772.Conclusions Patients with HFr EF complicated with pulmonary infection have lower immune function and elevated myocardial enzymes.The detection of immune indicators is helpful for the early diagnosis of HFr EF complicated with pulmonary infection.
