ORM1、cTnI与BNP联合检测在心力衰竭诊断中的应用价值探讨

摘　　要：目的 探讨α1-酸性糖蛋白(ORM1)、心肌肌钙蛋白(cTnI)与B型钠尿肽(BNP)联合检测对心力衰竭的诊断价值。方法 选取2020年1月至2021年4月我院收治的79例心力衰竭患者作为观察组,另选取同期在我院体检的50例健康者作为对照组。比较两组的ORM1、 cTnI、 BNP水平,并绘制各指标及联合检测因子Y诊断心力衰竭的受试者工作特性曲线(ROC),获得曲线下面积(AUC)评估其对心力衰竭的诊断价值。结果 观察组ORM1、 cTnI、 BNP水平均显著高于对照组(P<0.05)。心功能Ⅲ~Ⅳ级患者的ORM1、 cTnI、 BNP水平均高于心功能Ⅰ~Ⅱ级患者(P <0.05)。ROC曲线分析结果显示,ORM1、 cTnI、 BNP、联合检测因子Y对心力衰竭的AUC分别为0.617、 0.766、 0.813、 0.902,具有显著的诊断价值(P <0.05),其中联合检测的AUC高于任意单一指标。结论 ORM1、 cTnI、 BNP在心力衰竭诊断中的应用价值较高,各指标联合检测有利于提高心力衰竭的诊断效能。Objective To explore the diagnostic value of combined detection of α1-acid glycoprotein(ORM1),cardiac troponin(cTnI) and B-type natriuretic peptide(BNP) in the diagnosis of heart failure.Methods 79 patients with heart failure admitted to our hospital from January 2020 to April 2021 were selected as observation group,and 50 healthy people with physical examination in our hospital at the same period were selected as control group.The ORM1,cTnI and BNP levels were compared between the two groups.The receiver operating characteristic(ROC) curve of each indicator and combined detection factor Y in the diagnosis of heart failure was drawn,and the area under the curve(AUC) was obtained to evaluate the diagnostic value for heart failure.Results The ORM1,cTnI and BNP levels of the observation group were significantly higher than those of the control group(P <0.05).The ORM1,cTnI and BNP levels of patients with cardiac function grade Ⅲ to Ⅳ were higher than those of patients with cardiac function grade Ⅰ to Ⅱ(P <0.05).ROC curve analysis showed that the AUC of ORM1,cTnI,BNP and combined detection factor Y were 0.617,0.766,0.813 and 0.902 respectively,which had significant diagnostic value(P <0.05),and the AUC of combined detection factor was higher than any single indicator.Conclusions ORM1,cTnI and BNP have higher application value in the diagnosis of heart failure,and the combined detection of each index is beneficial to improve the diagnostic efficiency of heart failure.
