摘　　要：免疫重建不全(IIR)是部分艾滋病患者HAART治疗后出现的一种特殊的病理状态——低病毒载量和低免疫力,发病机制不明,临床治疗进入瓶颈期。中医倡导“扶正祛邪”“正气存内,邪不可干”,中医“扶助正气”之理囊括了“提高免疫力和免疫重建”的概念,免疫功能的恢复与否根本在于正气的强弱。近年来,研究者在中医药联合HAART治疗艾滋病IIR的临床疗效及机制研究方面均取得一些进展,治疗上体现了中医整体观念和扶正祛邪、阴阳平衡的思想和理论。同时文章对于中医药治疗艾滋病HAART后IIR的研究存在的问题和难题进行梳理思考,有利于克服难题,弥补不足,充分发挥中医药的作用和优势。Incomplete immune reconstitution(IIR)is a special pathological state after highly active antiretroviral therapy(HAART)treatment in some AIDS patients-low viral load and low immunity,the pathogenesis is unknown,and clinical treatment has entered a bottleneck period.Traditional Chinese medicine advocates‘supporting righteousness and eliminating evil’,‘righteous qi is stored inside,and evil cannot be done’.The principle of‘supporting righteous qi’in Chinese medicine encompasses the concept of‘improvement of immunity and immune reconstruction’.The restoration of immune function depends on the strength of righteousness.In recent years,researchers have made some progress in the clinical efficacy and mechanism research of Chinese medicine combined with HAART in the treatment of AIDS with IIR.The treatment embodies the overall concept of Chinese medicine,strengthening the body and eliminating evil,and the thought and theory of yin and yang balance.At the same time,the problems in the research of IIR after TCM treatment of AIDS and HAART will be sorted out and considered,which will help overcome the problems,make up for the shortcomings,and give full play to the role and advantages of TCM.
