基于“结者散之”衍生四法治疗冠心病经皮冠状动脉介入治疗术后再狭窄实证

摘　　要：经皮冠状动脉介入治疗(PCI)是目前国内外治疗冠心病的常用手段。但冠心病PCI术后再狭窄又是冠心病治疗面临的新难题。根据冠心病PCI术后再狭窄的临床症状,仍将其归属于“胸痹”的范畴。文章基于“结者散之”理论,总结出郁气内结、热毒内结、痰浊内结、瘀血内结皆是导致冠心病PCI术后再狭窄实证的病机关键。据此衍生出治疗冠心病PCI术后再狭窄实证的4种治法,即行气开郁散结法、清热解毒散结法、化痰软坚散结法、活血化瘀散结法,为临床治疗冠心病PCI术后再狭窄提供新的辨治思路和方法。Percutaneous coronary intervention(PCI)is a common method for the treatment of coronary heart disease at home and abroad.However,restenosis after PCI of coronary heart disease is a new problem facing the treatment of coronary heart disease.According to the clinical symptoms of restenosis after PCI of coronary heart disease,it still belongs to the category of‘chest obstruction’.Based on the theory of‘treating pathogenic accumulation with dissipation’,this paper concludes that the internal knot of stagnation of qi,internal knot of heat toxin,internal knot of turbid phlegm,and internal knot of blood stasis are the key pathogenesis of restenosis after PCI of coronary heart disease excess syndrome.Based on this,four treatment methods for the treatment of restenosis after PCI of coronary heart disease excess syndrome were derived,namely the method of promoting qi to open up the stagnation,the method of clearing away heat,detoxifying and dispelling knots,the method of relieving and softening phlegm and dispelling knots,and the method of promoting blood circulation and dispelling knots.It provides that new ideas and methods for the treatment of restenosis after PCI of coronary heart disease.
