摘  要:目的:揭示迎香穴埋线局部刺激经感觉神经网络系统P物质(SP)传递进一步调节鼻黏膜免疫效应细胞——肥大细胞(MC)而最终缓解变应性鼻炎(AR)的神经免疫调节机制。方法:36只SD雄性大鼠随机分为A组(AR组)、B组(AR穴位埋线组)、C组(AR辣椒素滴鼻组)、D组(空白对照组),除D组外其余各组以卵清蛋白(OVA)致敏制作AR模型。造模成功后A组OVA维持致敏,B组迎香穴埋线,C组辣椒素滴鼻。2周后,取4组大鼠鼻黏膜进行检测:①HE染色观察鼻黏膜炎症反应;②免疫组织化学法分析鼻黏膜SP积分光密度值;③甲苯胺蓝染色观察鼻黏膜MC数目;④Western Blot检测观察MC中β-类胰蛋白酶活性。最后对SP、MC数目及β-类胰蛋白酶行相关性分析。结果:A组鼻黏膜炎症反应、SP积分光密度值、MC数目及β-类胰蛋白酶均高表达,而D组均为低表达;B组与C组呈相似性表现,各项指标值介于A组和D组之间。SP与MC数量、SP与β-类胰蛋白酶呈强直线相关(P<0.01),MC数量与β-类胰蛋白酶呈中度相关(P=0.01)。结论:迎香穴埋线通过下调SP而减少MC数目、降低MC活性,进而改善AR鼻黏膜炎症,实现穴位埋线的局部神经刺激向鼻黏膜免疫调节的神经-免疫转换。Objective:To reveal the mechanism of neuroimmune regulation for mast cell(MC)by substance P(SP)in sensory neural network system through local stimulation of catgut implantation at Yingxiang(IL 20).Methods:Thirty-six male SD rats were randomly divided into group A(AR group),group B(acupoint catgut embedding group),group C(capsaicin nasal drip group)and group D(blank control group).The former three groups were sensitized with ovalbumin(OVA)except the group D.Then group A maintained sensitization by OVA nasal drip,group B received catgut embedding at Yingxiang(IL 20)point,and group C received capsaicin nasal drip.Two weeks later,nasal mucosa of rats in four groups were detected as follows:①HE staining was used to observe the inflammatory reaction of nasal mucosa;②Immunohistochemical method was used to analyze SP integral optical density of nasal mucosa;③Toluidine blue staining was used to observe the number of MC in nasal mucosa;④Western Blot was used to detectβ-tryptase.Finally,the correlation analysis of SP,MC number andβ-tryptase was carried out to reveal the link between SP and MC in nasal mucosa.Results:In group A,not only the inflammatory reaction but also the SP integral optical density value,MC number andβ-tryptase expression were all high,while these indicators showed low expression in group D.There was a similar performance between group B and group C,and the expression of the index values of these two groups were between group A and group D.SP-MC and SP-MCβ-tryptase were strongly linear correlation(P<0.01),but MC number was moderately correlated withβ-tryptase(P=0.01).Conclusion:Catgut embedding at Yingxiang(IL 20)acupoint could reduce the numbers of MC and decrease the activation of MC by down regulating SP,then improves the inflammation of AR nasal mucosa and promotes the neuroimmune transformation from local nerve stimulation to nasal mucosal immune regulation.
