摘  要:目的:通过对干眼兔泪腺组织高迁移率族蛋白B1(HMGB1)信号通路的研究,探讨针刺对干眼兔泪腺炎症的作用机制。方法:24只健康新西兰兔随机分为空白组、模型组、针刺组和西药组,每组6只。不同时间点空白组予0.9%氯化钠溶液皮下注射,其余3组予氢溴酸东莨菪碱皮下注射,2.0 mg·kg^(-1)·次^(-1),均连续35 d。造模第22天,西药组氟米龙滴眼液滴双眼;针刺组进行电针治疗,穴位为太阳穴、睛明、丝竹空、攒竹、瞳子髎,均双侧取穴,1次/d,留针15 min。检测4组泪液分泌量(SⅠT)和泪膜破裂时间(BUT),采用光镜对泪腺组织结构进行观察,采用免疫组化法检测泪腺HMGB1表达情况,采用Real-time PCR法检测泪腺HMGB1 mRNA表达情况。结果:治疗后,与模型组比较,针刺组和西药组SⅠT、BUT均显著改善(P<0.01);模型组泪腺柱状上皮明显萎缩,分泌减少,可见淋巴细胞浸润,而西药和针刺可改善泪腺柱状上皮细胞萎缩;与空白组比较,模型组泪腺组织中HMGB1蛋白和mRNA表达显著升高(P<0.01);与模型组比较,针刺组和西药组泪腺组织中HMGB1蛋白和mRNA表达显著降低(P<0.01)。结论:通过针刺干预可调节干眼兔SⅠT、BUT,调节泪腺功能,其机制可能与调节泪腺组织中HMGB1的表达有关。Objective:To investigate the mechanism of acupuncture on inflammation of lacrimal gland in dry eye rabbits by studying the high mobility group box 1(HMGB1)signal pathway.Methods:A total of 24 healthy New Zealand rabbits were randomly divided into blank group,model group,acupuncture group and western medicine group,with 6 rabbits in each group.Blank group:subcutaneous saline injection was performed every day,the other three groups were given scopolamine hydrobromide 2.0 mg·kg^(-1)·time^(-1),subcutaneous injection,all for 35 days.The 22nd day of modeling,western medicine group:fluorometholone eye drops on both eyes;acupuncture group:electroacupuncture treatment,acupoints:Taiyang(EX-HN 5),Jingming(BL 1),Sizhukong(SJ 23),Cuanzhu(BL 2),Tongziliao(GB 1),both sides were selected,once a day,needle retention for 15 minutes.The SchirmerⅠtest(SⅠT)and tear film breakup time(BUT)were measured.The lacrimal gland tissue structure was observed by light microscope.The expression of HMGB1 in lacrimal gland was detected by immunohistochemistry.The expression of HMGB1 mRNA in lacrimal gland was detected by Real-time PCR.Results:After treatment,compared with the model group,SⅠT and BUT in the acupuncture group and western medicine group were significantly improved(P<0.01);the columnar epithelium of lacrimal gland in the model group was significantly atrophied,the secretion was reduced,and lymphocyte infiltration was observed,while western medicine and acupuncture group could improve the atrophy of lacrimal gland columnar epithelial cells.Compared with the blank group,the expression of HMGB1 protein and mRNA in the lacrimal gland tissue of the model group was significantly increased(P<0.01).Compared with the model group,the expression of HMGB1 protein and mRNA in lacrimal gland of acupuncture group and western medicine group were significantly decreased(P<0.01).Conclusion:Acupuncture can regulate SⅠT,BUT and lacrimal gland function in dry eye rabbits.The mechanism may be related to the regulation of HMGB1 expression in lacri
