苗药熏蒸外用联合激痛点针刺治疗风寒湿型冻结肩临床观察

摘　　要：目的:观察苗药熏蒸外用联合激痛点针刺治疗风寒湿型冻结肩(FS)临床疗效。方法:将80例风寒湿型FS患者随机分为治疗组和对照组,各40例。治疗组苗药熏蒸外用联合激痛点针刺治疗,对照组采用双氯芬酸二乙胺乳胶剂外用联合痛点封闭治疗;观察两组治疗前后肩关节活动度(ROM)以评价肩关节活动范围的变化,观察视觉模拟评分(VAS)以评定患侧肩关节疼痛状况,观察临床疗效,通过电话随访考察复发率情况。结果:与治疗前比较,两组患者治疗后VAS评分和ROM评分均显著改善(P<0.05);且治疗组内旋活动方面改善优于对照组(P<0.05);治疗组有效率高于对照组,但差异无统计学意义;治疗组患者在治疗后4个月的复发率显著低于对照组(P<0.05)。结论:苗药熏蒸外用联合激痛点治疗FS患者,在缓解疼痛和改善肩关节功能活动等方面均有较好的临床疗效,与双氯芬酸二乙胺乳胶剂外用联合痛点封闭治疗相比,更具有不易复发、疗效更持久等优点。Objective:To observe the clinical effect of treatment of wind-cold and dampness frozen shoulder(FS)by external fumigation of vaccine and medicine combined with acupoint acupuncture.Methods:A total of 80 wind-cold and dampness FS patients were randomly divided into treatment group and control group,40 cases in each group.The treatment group was treated with fumigation of vaccine drugs combined with stimulated pain point acupuncture(MTRP)acupuncture,while the control group was treated with diclofenac diethylamine latex combined with pain point blocking.The ROM of the two groups before and after treatment was observed to evaluate the changes in the range of motion of the shoulder joint,the visual analog scale(VAS)was observed to evaluate the pain of the affected side of the shoulder joint and clinical efficacy,and the recurrence rate was investigated by telephone follow-up.Results:Compared with before treatment,VAS score and ROM score in two groups were significantly improved after treatment(P<0.05),the improvement of internal rotation activity in the treatment group was better than that in the control group(P<0.05).The efective rate of the treatment group was higher than that of the control group,but the difference was not statistically significant.The recurrence rate in treatment group was significantly lower than that in control group 4 months after treatment(P<0.05).Conclusion:The treatment of frozen shoulder patients with external fumigation of vaccine drugs combined with MTRP has good clinical efficacy in relieving pain and improving functional activities of shoulder joint.Compared with the treatment of extermal application of diclofenac diethylamine latex combined with pain point sealing,it has advantages of less recurrence and more lasting efficacy.
