肋骨骨折实施多层螺旋CT三维重建与DR胸片诊断的准确性对比分析

摘　　要：目的:分析肋骨骨折患者的临床诊断中采用数字X射线摄影(digital radiography,DR)和多层螺旋CT(multislice spiral CT,MSCT)三维重建的诊断准确性。方法:选取2019年1月—2021年12月期间在泰州市第二人民医院就诊的52例疑似肋骨骨折患者,所有患者均进行DR、MSCT检查,并对手术结果实施追踪和随访,对2种诊断方法的诊断效果实施比较和分析。结果:通过追踪和随访发现,52例疑似患者中,发现肋骨骨折38例,非肋骨骨折14例。肋骨骨折的38例患者中,骨折共有64处,其中8处为隐匿性骨折,15处为多发性骨折,41处为单发性骨折;MSCT诊断肋骨骨折结果方面,隐匿性的为100.00%,多发性的为100.00%,单发性的为97.56%,总检出率为98.44%,均高于DR诊断的50.00%、66.67%、80.49%、73.44%,组间对比差异显著(P<0.05);MSCT诊断的敏感度为97.37%,特异度为92.86%,准确性为96.15%,均高于DR诊断的76.32%、78.57%、76.92%,组间对比差异显著(P<0.05)。结论:在肋骨骨折患者的临床诊断中,与DR相比,多层螺旋CT的诊断价值更高,可有效检出肋骨骨折,为临床诊疗提供可靠的依据,推广价值也更高。Objective To analyze the diagnostic accuracy of three-dimensional reconstruction of digital radiography(DR)and multislice spiral CT(MSCT)in the clinical diagnosis of rib fracture.Methods The analysis subjects were 52 patients with suspected rib fracture admitted to Taizhou Second People's Hospital from January 2019 to December 2021.All patients were examined and diagnosed by DR and MSCT,and the surgical results were tracked and followed up.The diagnostic effects of the two diagnostic methods were compared and analyzed.Results Through tracking and follow-up,among 52 suspected patients,38 rib fractures and 14 non-rib fractures were found.Among the 38 patients with rib fractures,there were 64 fractures,of which 8 were occult fractures,15 were multiple fractures,and 41 were single fractures;The CT diagnosis of rib fractures showed occult fractures.The total detection rate was 98.44%,which were significantly higher than 50.00%,66.67%,80.49%and 73.44%of DR diagnosis(P<0.05).The sensitivity of CT diagnosis was 97.37%,the specificity was 92.86%,and the accuracy was 96.15%,which were significantly higher than 76.32%,78.57%and 76.92%of DR diagnosis,the difference between groups was significant(P<0.05).Conclusion Compared with DR,in the clinical diagnosis of rib fractures,multi-slice spiral CT has a higher diagnostic value,can effectively detect rib fractures,provide a reliable basis for clinical diagnosis and treatment,and has a higher promotion value.
