摘　　要：目的:探究临床药师运用4R危机管理方法干预医保药品目录限定支付的效果。方法:收集江西医学高等专科学校第一附属医院医保限定支付目录药品的医嘱作为本次研究的资料,对2020年5月至2021年2月运用4R危机管理方法干预前与2021年3月至2021年12月运用4R危机管理方法干预后的医保限定支付药品使用情况进行对比。分析干预前后医保限定支付药品使用情况、医保限定支付药品不合理医嘱情况、临床药师总满意率。结果:干预后医保限定支付药品不合理医嘱发生率、使用总金额有所下降,临床药师总满意率高于干预前,差异有统计学意义(P<0.05)。结论:临床药师运用4R危机管理方法对医保药品目录限定支付进行干预效果显著,能有效地减少医保限定支付药品不合理医嘱的发生风险,且临床药师对该种模式满意度较高,能够为患者的合理用药提供安全保障。Objective:To explore the effect of clinical pharmacists using 4R crisis management method to intervene in the restricted payment of medical insurance drug list.Methods:Collect the medical orders of the medicare restricted payment list drugs in The First Affiliated Hospital of Jiangxi Medical College as the research data of this study,and compare the use of Medicare restricted payment drugs before the intervention with 4R crisis management method from May 2020 to February 2021 and after the intervention with 4R crisis management method from March 2021 to December 2021.Analysis was made on drug use,unreasonable medical orders and total satisfaction rate of clinical pharmacists before and after intervention.Results:After the intervention,the incidence of unreasonable medical orders and the total amount of drug use decreased,and the total satisfaction rate of clinical pharmacists was higher than before the intervention,with statistical significance(P<0.05).Conclusions:The intervention effect of clinical pharmacists using 4R crisis management method on the limited payment of medical insurance drugs list is significant,which can effectively reduce the risk of unreasonable medical orders of drugs limited payment of medical insurance,and clinical pharmacists have high satisfaction with this mode,which can provide safety guarantee for rational drug use of patients.
