摘  要:目的:探讨妊娠晚期脐动脉S/D比值反复升高对子代出生及生长发育的影响。方法:选取2019年1月—12月于嘉兴市妇幼保健院分娩的孕产妇,将妊娠晚期脐动脉S/D比值反复升高的单胎孕产妇125例作为研究组,将同期妊娠晚期脐动脉S/D比值正常的单胎孕妇125例作为对照组,收集两组子代出生及生长发育情况,经统计学软件进行数据分析。结果:研究组阴道试产转剖宫产、剖宫产、脐带绕颈、早产、胎儿生长受限、胎儿宫内窘迫、新生儿窒息、入新生儿监护室发生率均高于对照组,差异均有统计学意义(P<0.05);研究组胎盘体积、胎盘重量、新生儿出生体质量低于对照组(t=3.188、11.612、10.241,P<0.05);两组子代12月龄体格Z评分组间差异无统计学意义(P>0.05)。研究组沟通能区、精细动作能区、解决问题能区、个人-社会能区得分均低于对照组(t=5.768、3.776、2.168、3.901,P<0.05);两组大动作能区得分组间差异无统计学意义(t=1.625,P>0.05)。结论:妊娠晚期脐动脉S/D比值升高对子代出生及子代神经认知和行为发育可能有不良影响。Objective To investigate the effect of elevated S/D ratio of umbilical artery in late pregnancy on the birth and growth of offspring.Methods Pregnant women who gave birth in our hospital from January to December 2019 were selected,125 singleton pregnant women whose umbilical artery S/D ratio repeatedly increased in the third trimester of pregnancy were selected as the research group,and 125 singleton pregnant women with normal S/D ratio were used as the control group.The birth,growth and development of the offspring of the two groups were collected,and the data were analyzed by statistical software.Results The incidence of vaginal trial delivery to cesarean section,cesarean section,umbilical cord wrapping around the neck,premature birth,fetal growth restriction,fetal distress,neonatal asphyxia,and admission to the neonatal intensive care unit in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).The placenta volume,placental weight,neonatal birth weight in the study group were lower than those in the control group(t=3.188,11.612,10.241,P<0.05).There was no significant difference in the 12-month-old physique Z-score of the offspring between the groups(P>0.05).The scores of communication,fine motor,problem-solving,and personal-social skills in the study group were lower than those in the control group(t=5.768,3.776,2.168,3.901,P<0.05),and the gross motor skills between groups was not statistically significant(t=1.625,P>0.05).Conclusion The increased S/D ratio of umbilical artery in late pregnancy may have adverse effects on offspring birth and neurocognitive and behavioral development of offspring.
