摘 要:目的了解HIV感染者外周血滤泡辅助性T细胞(circulating T follicular helper cell,cTfh)比例与血浆IgG水平的相关性。方法选取未经抗病毒治疗(antiretroviral therapy,ART)的慢性期HIV感染者(chronically HIV infected individuals,CHI),同时筛选HIV抗体阴性的高危暴露者(HIV-exposed seronegative individuals,HESN)作为对照。采用多色流式分析方法,分别检测CHI和HESN的cTfh、ICOS^(+)cTfh、CD40L^(+)cTfh和浆母细胞(plasmablasts,PB)比例。通过免疫散射比浊法检测血浆IgG浓度。结果与HESN相比,CHI组ICOS^(+)cTfh比例(26.97±1.62 vs.18.83±0.99,t=4.518,P<0.01)、PB比例(1.59±0.32 vs.0.46±0.06,t=3.982,P<0.01)和血浆IgG浓度(14.71±0.93 vs.10.65±0.52 g/L,t=4.064,P<0.01)均显著升高。CHI组ICOS^(+)cTfh比例(r=0.586,P=0.030)和PB比例(r=0.372,P=0.038)均与血浆IgG浓度正相关。结论HIV感染导致ICOS^(+)cTfh的增殖可能是引起血浆IgG过度分泌的因素之一,从而引发高丙种球蛋白血症。Objective To understand the correlation between the proportion of circulating T follicular helper cell(cTfh)and the level of plasma IgG in HIV infected patients.Methods Chronically HIV infected subjects(CHI)without antiretroviral therapy(ART)were enrolled and HIV-exposed seronegative individuals(HSEN)were screened as the control.Multicolor flow cytometry was used to analyze the proportions of cTfh,ICOS^(+)cTfh,CD40L^(+)cTfh and plasmablasts(PB)in CHI and HSEN.The level of plasma IgG was determined by turbidimetric inhibition immunoassay.Results Compared to HSEN,the percentage of ICOS^(+)cTfh(26.97±1.62 vs.18.83±0.99,t=4.518,P<0.001),PB(1.59±0.32 vs.0.46±0.06,t=3.982,P<0.001),as well as the level of plasma IgG(14.71±0.93 vs.10.65±0.52 g/L,t=4.064,P<0.001)elevated significantly in CHI.The percentages of ICOS^(+)cTfh(r=0.586,P=0.030)and PB(r=0.372,P=0.038)were positively correlated to the level of plasma IgG in CHI.Conclusions The proliferation of ICOS^(+)cTfh cells caused by HIV infection may be one of the factors related to the over secretion of plasma IgG,and consequently causes hypergammaglobulinemia.
