血清热休克蛋白27与冠心病的相关性研究

摘　　要：目的分析血清热休克蛋白27(HSP27)水平与冠心病的相关性。方法选取2020年7月至2021年8月三门峡市中心医院收治的300例心内科住院患者,其中冠状动脉造影结果正常者60例为对照组,冠心病患者240例,按临床诊断分型进行分组,急性心肌梗死组81例,不稳定型心绞痛组74例,稳定型心绞痛组85例,测定各组患者血清HSP27水平。结果急性心肌梗死组血清HSP27水平高于对照组(P<0.001),冠心病各组血清HSP27水平相比,差异无统计学意义(P>0.05)。三支病变组血清HSP27水平高于对照组(P<0.05),单支、双支、三支病变组血清HSP27水平比较,差异无统计学意义(P>0.05)。经logistic回归分析,血清HSP27是冠心病的保护性因子(P<0.05),高血压、高脂血症、超重、糖尿病、睡眠呼吸暂停综合征、C反应蛋白是冠心病的危险因素(P<0.05)。经Pearson相关分析,HSP27与体质量指数(r=-0.437)、C反应蛋白(r=-0.533)、总胆固醇(r=-0.615)、低密度脂蛋白(r=-0.503)、甘油三酯(r=-0.206)、血糖(r=-0.523)呈负相关(P<0.05),与高密度脂蛋白呈正相关(r=0.642,P<0.05)。结论急性心肌梗死及冠状动脉三支病变患者血清HSP27水平增高,血清HSP27是冠心病的保护性因子。Objective To evaluate the relationship between heart shock protein 27(HSP27)and coronary heart disease(CHD).Methods A total of 300 inpatients in Department of Cardiology of Central Hospital of Sanmenxia from July 2020 to August 2021 were selected.Among them,60 patients with normal coronary angiography results were the control group,and 240 patients with coronary heart disease were grouped according to clinical diagnosis.There were 81 patients in acute myocardial infarction group,74 patients in unstable angina pectoris group and 85 patients in stable angina pectoris group.The serum HSP27 levels of patients in each group were measured.Results The level of serum HSP27 in acute myocardial infarction group was higher than that in control group(P<0.001).There was no statistical difference in serum HSP27 levels among the coronary heart disease groups(P>0.05).The serum HSP27 level in three vessel lesion group was higher than that in control group(P<0.05).There was no statistical difference in serum HSP27 levels among single vessel,double vessel and three vessel lesion groups(P>0.05).Logistic regression analysis showed that serum HSP27 was the protective factor of coronary heart disease(P<0.05),and hypertension,hyperlipidemia,overweight,diabetes,sleep apnea syndrome and C-reactive protein were the risk factors of coronary heart disease(P<0.05).According to Pearson correlation analysis,HSP27 was negatively correlated with body mass index(r=-0.437),C-reactive protein(r=-0.533),total cholesterol(r=-0.615),low density lipoprotein(r=-0.503),triglyceride(r=-0.206)and blood glucose(r=-0.523)(P<0.05),and positively correlated with high density lipoprotein(r=0.642,P<0.05).Conclusion The level of serum HSP27 was increased in patients with acute myocardial infarction and three branch coronary artery disease.Serum HSP27 was a protective factor of coronary heart disease.
