摘　　要：目的分析闭合复位支架外固定术对桡骨远端骨折患者围手术期情况、骨折愈合时间及术后腕关节主动活动度的影响。方法回顾性收集遂平县中医院92例桡骨远端骨折患者的临床资料,根据手术方式分为A组(切开复位锁定加压钢板内固定术,43例)和B组(闭合复位支架外固定术,49例)。比较两组手术时间、术中出血量、骨折愈合时间、术后并发症及术后不同时间腕关节主动活动度的情况。结果B组术中出血量少于A组(P<0.05);B组手术时间和骨折愈合时间短于A组(P<0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。B组术后3个月腕关节主动活动度大于A组(P<0.05);术后1 a,两组腕关节主动活动度比较,差异无统计学意义(P>0.05)。结论闭合复位支架外固定术应用于桡骨远端骨折患者可有效缩短手术时间及骨折愈合时间,减少术中出血量,提高近期腕关节主动活动度,可避免二次手术。Objective To analyze the influence of closed reduction and external fixation with stent on the perioperative situation,fracture healing time and active activity of wrist joint in patients with distal radius fracture.Methods The clinical data of 92 patients with distal radius fracture in Suiping Hospital of Traditional Chinese Medicine were retrospective collected.According to the operation methods,they were divided into group A(open reduction and locking compression plate internal fixation,43 cases)and group B(closed reduction and external fixation with stent,49 cases).The operation time,intraoperative hemorrhage,fracture healing time,postoperative complications and active mobility of wrist joint were compared between the two groups.Results The amount of bleeding in group B was less than that in group A(P<0.05).The operation time and fracture healing time in group B were shorter than those in group A(P<0.05).There was no statistical difference in the incidence of postoperative complications between the two groups(P>0.05).The active range of motion of wrist joint in group B was greater than that in group A 3 months after operation(P<0.05).At 1 year after operation,there was no statistical difference in the active range of wrist motion between the two groups(P>0.05).Conclusion Closed reduction and external fixation with stent can effectively shorten the operation time and fracture healing time in patients with distal radius fractures,reduce intraoperative bleeding,improve the recent active activity of wrist joint and avoid secondary operation.
