摘 要:目的多维效能分析透射比浊法及散射比浊法测定血清总IgE与荧光酶免疫法(ImmunoCAP法)测定血清总IgE的相关性和一致性,为临床进行不同检测方法间的相互参照提供支持。方法取184例9个月到13岁患儿血清标本,全部采用荧光酶免疫法(ImmunoCAP法,德国赛默飞Phadia100)测定总IgE,其中108例样本同时应用散射比浊法测定总IgE,76例样本应用透射比浊法测定总IgE,对两组数据进行相关分析、等级分组一致性分析,以及对散射比浊法组进行随机敏感性分析。结果相关性分析发现,透射比浊法与荧光酶免疫法结果具有高度相关性(Spearman相关系数为0.982,P<0.001),等级分组一致性良好(κ值为0.774,P<0.001)。散射比浊法与荧光酶免疫法结果具有高度相关性(Spearman值为0.955,P<0.001),等级分组一致性良好(κ值为0.766,P<0.001)。且透射比浊法在不同浓度等级组都与荧光酶免疫法有较紧密的相关性。结论透射比浊法或散射比浊法与荧光酶免疫法检测血清总IgE具有高度一致性,当荧光酶免疫法无法获得时,比浊法得出的结果可做参照。Objective To perform multi-dimensional efficiency analysis for evaluation of correlation and consistency among serum total immunoglobulin E(IgE)concentrations detected by turbidimetry,nephelometry,and fluorescence enzymatic immunoassay(ImmunoCAP,Thermo Fisher Phadia100,Germany)methods,providing evidence for cross-reference among different detection methods in clinical practice.Methods Serum samples of 184 children aged from 9 month-old to 13 year-old were collected and total IgE concentrations of all samples were determined by fluorescence enzymatic immunoassay(ImmunoCAP).Meanwhile,total IgE levels of 108 samples in same population were also detected by nephelometry method and another 76 samples were detected by turbidimetry method.The consistency analysis of graded data and correlation analysis were performed in both nephelometry and turbidimetry group,while stochastic sensitivity analysis was performed in nephelometry group.Results The results of correlation analysis demonstrated that results of turbidimetry method were highly correlated with that of fluorescence enzyme immunoassay(ImmunoCAP)(Spearman correlation coefficient 0.982,P<0.001),which was also confirmed by results of graded data analysis(κ=0.774,P<0.001).The results of nephelometry were also highly correlated with that of fluorescence enzyme immunoassay(Spearman correlation coefficient 0.955,P<0.001),which was also confirmed by results of graded data analysis(κ=0.766,P<0.001).Moreover,the results of turbidimetric method were closely correlated with that of fluorescence enzyme immunoassay among different concentration subgroups.Conclusions Either the results of turbidimetry method or the results of nephelometry method are highly consistent with that of fluorescence enzyme immunoassay(ImmunoCAP)for detection of total serum IgE.When fluorescence enzyme immunoassay is not available,the results of turbidimetric method could be used as reference.
