腹部推拿治疗神经源性尿潴留的作用机制研究

摘　　要：骶髓损伤是尿潴留的重要病因,目前,治疗尿潴留方法包括手术治疗、连续或间接导尿等方式,可减少残余尿量,从而减轻症状,但给患者带来极大痛苦,且不能从根本上治疗尿潴留。腹部推拿是推拿手法的重要分支,也是尿潴留常用的治法之一,可通过AMPK/mTOR通路激活自噬实现对神经源性尿潴留大鼠的保护作用,有可能是通过激活自噬而对膀胱的排尿功能进行改善,为临床上腹部推拿治疗神经源性尿潴留提供一定参考依据。目前,关于腹部推拿的作用机制研究尚不充分,自噬与神经源性尿潴留之间关系尚需进一步研究。今后,应加强对自噬与神经源性尿潴留关系的研究,阐释腹部推拿对人体功能调控的作用机制,为临床应用提供理论依据。Sacral spinal cord injury is an important cause of urinary retention.At present,the treatments of urinary retention include surgical treatment,continuous or indirect catheterization and other methods,which can reduce residual urine volume and relieve symptoms,but it brings great pain to patients and cannot fundamentally treat urinary retention.Abdominal massage is an important branch of massage manipulation,and it is also one of the commonly used treatments for urinary retention.Autophagy can be activa-ted through AMPK/mTOR pathway to achieve the protective effect on rats with neurogenic urinary retention,which may be to im-prove the micturition function of bladder by activating autophagy,so as to provide a certain reference basis for clinical upper ab-dominal massage in the treatment of neurogenic urinary retention.At present,the mechanism of abdominal massage is not fully studied,and the relationship between autophagy and neurogenic urinary retention needs further study.In the future,it is necessary to strengthen the research on the relationship between autophagy and neurogenic urinary retention,and explain the mechanism of abdominal massage on the regulation of human function,so as to provide a theoretical basis for clinical application.
