后纵韧带切除与否对人工颈椎间盘置换手术疗效的影响

摘　　要：目的探讨和对比颈椎人工椎间盘置换手术中颈后纵韧带(PLL)是否切除的临床疗效,及切除后对颈椎活动度的具体影响,为临床颈椎病的治疗提供参考。方法对安阳市人民医院2015年2月至2018年2月期间因颈椎病接受Bryan颈椎人工椎间盘置换手术的64例颈椎椎间盘突出患者的临床资料进行回顾性分析,并且根据患者术中是否切除颈PLL分为颈PLL切除组(32例)和颈PLL保留组(32例),比较两组患者的一般情况、临床疗效、影像学结果及术后并发症的发生状况。结果经过24个月的术后观察后发现:颈PLL切除组的术后JOA评分显著高于颈PLL保留组,差异有统计学意义(P<0.05)。两组患者矢状位及左右侧屈位活动度均有增加,但差异不具有统计学意义(P>0.05)。仅颈PLL保留组患者出现1例肩颈部疼痛,两组患者术后并发症发生率不具有统计学意义(P>0.05)。结论切除颈椎PLL在提高颈椎人工椎间盘置换患者的临床疗效中有较好的效果,且并发症发生几率小,故术中对患者颈PLL进行切除是安全可行的。Objective To investigate and compare the clinical efficacy of cervical posterior longitudinal ligament(PLL)resection in cervical artificial disc replacement,and the specific impact on cervical mobility after resection,so as to provide reference for the treatment of clinical cervical spondylosis.Methods the clinical data of 64 patients with cervical disc herniation who underwent Bryan cervical artificial disc replacement surgery for cervical spondylosis in Anyang People's Hospital from February 2015 to February 2018 were retrospectively analyzed,and they were divided into cervical PLL resection group(32 cases)and cervical PLL retention group(32 cases)according to whether the cervical PLL was removed during surgery.The general conditions,clinical efficacy,imaging results and postoperative complications of the two groups were compared.Results After 24 months of postoperative observation,it was found that the JOA score of the neck PLL resection group was significantly higher than that of the neck PLL retention group(P<0.05).The sagittal and lateral flexion mobility of the two groups increased,but the difference was not statistically significant(P>0.05).There was only one case of shoulder and neck pain in the neck PLL retention group,and the incidence of postoperative complications in the two groups was not statistically significant(P>0.05).Conclusion Resection of cervical PLL has a good effect in improving the clinical efficacy of cervical artificial disc replacement,and the incidence of complications is small,so it is safe and feasible to resection of cervical PLL during operation.
