摘　　要：目的:探讨表面缝盖脱细胞异体真皮基质 (Acellular dermal matrix,ADM)口腔黏膜组织补片对颊黏膜缺损修复的效果及预后。方法:对笔者科室2016年6月-2018年6月应用表面缝盖ADM修复颊黏膜缺损的35例患者临床资料进行回顾性分析,评估患者修复效果、手术前后缺损创面直径及张口度、患者满意度及口腔健康影响程度量表(OHIP-14)评分等。结果:35例患者均手术成功,其中ADM完全成活32例(91.43%),部分成活3例(8.57%),无修复失败病例;术后2周、3个月患者缺损创面直径较术前更小,张口度较术前更高,差异有统计学意义(P<0.05),术后3个月,患者缺损创面直径较术后2周更低,张口度较术后2周更高,差异有统计学意义(P<0.05)。患者满意度可达91.43%,末次随访时患者OHIP-14评分较术前明显更低,差异有统计学意义(P<0.001)。结论:颊黏膜缺损患者应用表面缝盖ADM进行修复效果好,皮瓣成活率较高,患者满意度好,并可改善患者生活质量,值得临床推广应用。Objective To explore the effect and prognosis of the surface gap acellular allogeneic dermal matrix (ADM) oral mucosal tissue patch on the repair of buccal mucosal defects.Methods A retrospective analysis of the clinical data of 35 patients in the author’s department who used surface slit cover ADM to repair buccal mucosal defects from June 2016 to June 2018,To evaluate the effect of patient repair,the diameter and opening of the defect wound before and after surgery,patient satisfaction,oral health impact scale (OHIP-14) score,etc.Results All 35 patients were successfully operated on,of which 32 cases (91.43%) fully survived ADM and 3 cases (8.57%) partially survived.There was no recovery failure.2 weeks and 3 months after the operation,the diameter of the defect wound was smaller than before the operation,and the mouth opening was higher than before the operation,the difference was statistically significant (P<0.05),the wound diameter of the defect at 3 months after operation was lower than that at 2 weeks after operation,and the mouth opening was higher than that at 2 weeks after operation,the difference was statistically significant (P<0.05).Patient satisfaction can reach 91.43%.The patient’s OHIP-14 score at the last follow-up was significantly lower than that before the operation,and the difference was statistically significant (P<0.001).Conclusion For patients with buccal mucosal defects,the application of surface suture cover ADM has good repair effect,high flap survival rate,good patient satisfaction,and can improve the quality of life of patients.It is worthy of clinical application.
