摘  要:目的研究糖尿病肾病(DN)患者外周血长链非编码(lncRNA)PACER表达与炎症反应及肾功能进展的关系。方法选择2018年1月至2020年5月期间在石家庄市第二医院就诊的72例DN患者作为DN组、80例2型糖尿病患者作为DM组、88例健康者作为对照组,检测外周血lncRNA PACER的表达水平及血清肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、细胞间黏附分子-1(ICAM-1)的含量。根据DN患者的24 h尿白蛋白水平分为微量白蛋白尿(30~300 mg/24 h)为和大量蛋白尿(>300 mg/24 h),根据估算内生肌酐清除率分为慢性肾脏病(CKD)1期、2期、3期,随访DN患者肾功能不良进展的累积发生情况。结果各组外周血中lncRNA PACER表达水平及血清中TNF-α、IL-1β、ICAM-1水平比较:DN组>DM组>对照组,差异有统计学意义(F=14.822,23.811,17.582,19.508,P<0.05)。DN组中大量白蛋白尿患者高于微量白蛋白尿患者,差异有统计学意义(t=2.215,9.300,6.257,10.018,P<0.05)。DN组中CKD1期患者<CKD2期患者<CKD3患者,差异有统计学意义(F=14.952,22.812,19.382,26.842,P<0.05);DN组中lncRNA PACER高表达患者的肾功能不良进展累积发生率高于lncRNA PACER低表达患者(P<0.05);lncRNA PACER表达水平对DN患者肾功能不良进展具有预测价值。结论外周血lncRNA PACER高表达与DN病情加重、炎症反应激活及肾功能不良进展有关。Objective To study the relationship between long non encoding RNA(lncRNA)PACER expression in peripheral blood and inflammatory response,renal function progression in patients with diabetic nephropathy(DN).Methods 72 cases of DN patients in our hospital from January 2018 to May 2020 were selected as the DN group and 80 cases of type 2 diabetic patients as the DM group and 88 cases of healthy persons as the control group.The expression of lncRNA PACER in peripheral blood and the contents of tumor necrosis factor⁃α(TNF⁃α),interleukin⁃1β(IL⁃1β),intercellular adhesion molecule⁃1(ICAM⁃1)in serum were detected.According to the 24 h urinary albumin level of DN patients,they were divided into microalbuminuria(30-300 mg/24 h)and massive proteinuria(>300 mg/24 h).According to the estimated endogenous creatinine clearance,they were divided into chronic kidney disease(CKD)stage 1,stage 2 and stage 3.The cumulative incidence of renal dysfunction in patients with DN was followed up.Results The change trend of expression level of lncRNA PACER in peripheral blood and the contents of TNF⁃α,IL⁃1β,ICAM⁃1 in serum were:DN group>DM group>control group(F=14.822,23.811,17.582,19.508,P<0.05).Massive albuminuria patients were higher than microalbuminuria patients in the DN group(t=2.215,9.300,6.257,10.018,P<0.05).CKD1 stage patient<CKD2 stage patient<CKD3 stage patient in the DN group(F=14.952,22.812,19.382,26.842,P<0.05).The cumulative incidence of renal dysfunction in patients with high expression of ln⁃cRNA PACER in the DN group was higher than that in patients with low expression of lncRNA PACER(P<0.05).The expression level of lncRNA PACER has predictive value for the progression of renal dysfunction in patients with DN.Conclusion The high expression of lncRNA PACER in peripheral blood is associated with the aggravation of DN,activation of inflammatory response and the progression of renal dysfunction.
