摘 要:目的探讨免疫球蛋白、高敏C-反应蛋白(hs-CRP)联合检测传染性单核细胞增多症(IM)诊断价值。方法选取2017年1月至2021年12月铜陵市人民医院儿科102例IM患儿作为研究组,另纳入48例支气管肺炎患儿作为对照组。统计两组及研究组不同异型淋巴细胞百分比(ALY%)水平患儿免疫球蛋白、hs-CRP,Spaerman分析ALY%与免疫球蛋白、hs-CRP相关性,绘制受试者工作特征曲线(ROC)分析各指标诊断效能,采用相对危险度(RR)及其95%CI分析各指标对IM发生危险度的影响。结果研究组血清IgA、IgM、IgG、hs-CRP水平高于对照组,差异有统计学意义(t=6.789,6.725,6.362,7.173,P<0.05);研究组ALY%≥10%患儿IgA、IgM、IgG、hs-CRP水平高于ALY%<10%患儿,差异有统计学意义(t=2.435,4.034,5.252,2.999,P<0.05);Spaerman相关性显示,血清IgA、IgM、IgG、hs-CRP水平与ALY%呈正相关(r=0.551,0.634,0.598,0.546,P均<0.05);四者联合诊断IM的AUC优于单独诊断(P<0.05);IgA、IgM、IgG、hs-CRP高水平患儿发生IM的RR分别为低水平1.435倍、1.466倍、1.510倍、1.628倍。结论IM儿童免疫球蛋白、hs-CRP高表达与ALY%有关,联合检测有助于提高诊断价值,为后续诊治工作开展提供科学依据。Objective To investigate the diagnostic value of the combined immunoglobulin and high-sensitivity C-reactive protein(hs-CRP)test for infectious mononucleosis(IM). Methods 102 children with IM in the Pediatric Department of Tongling People’s Hospital from January 2017 to December 2021 were selected as the study group,and another 48 children with bronchopneumonia were included as the control group. Immunoglobulins and hs-CRP of children with different levels of allogeneic lymphocytes(ALY%)in both groups and the study group were counted,and Spearman analyzed the correlation between ALY% and immunoglobulins and hs-CRP,and the diagnostic efficacy of each index was analyzed by drawing the subject operating characteristic curve(ROC),and the relative risk ratio(RR)and its 95% CI were used to analyze the risk of each indicator on the occurrence of IM. The influence of the indicators on the risk of IM occurrence was analyzed using relative risk ratio(RR)and its 95% CI. Results The Serum IgA,IgM,IgG,and hs-CRP levels were higher in the study group than those in the control group,with statistically significant differences(t=6.789,6.725,6.362,7.173,P<0.05);IgA,IgM,IgG,and hs-CRP levels were higher in children with ALY% ≥ 10% than in children with ALY% < 10% in the study group,with statistically significant differences(t=2.435,4.034,5.252,2.999,P<0.05). Spearman correlation showed that serum IgA,IgM,IgG,and hs-CRP levels were positively correlated with ALY%(r=0.551,0.634,0.598,0.546,P<0.05). The AUC of the combined diagnosis of IM by the four was better than that of the diagnosis alone(P<0.05). The RR of IM occurring in children with high levels of IgA,IgM,IgG,and hs-CRP was 1.435,1.466,1.510,and 1.628 times higher than that with low levels,respectively. Conclusion The high expression of immunoglobulin and hs-CRP in children with IM is related to ALY%. Ther combined detection can help to improve the diagnostic value and provide a scientific basis for the follow-up diagnosis and treatment.
