膀胱功能重建干预对膀胱癌手术患者膀胱功能、生活质量的影响研究

摘　　要：目的:研究膀胱功能重建干预的实施方法及其在膀胱癌患者术后护理中的应用价值。方法:将某院收治的108例膀胱癌手术患者随机分为A组和A+P组各54例,A组采用常规护理,A+P组在A组的基础上联合采用膀胱功能重建护理(主要包括积极预防术后并发症、提高膀胱顺应性、膀胱功能康复锻炼等)。于护理前后,分别检测膀胱功能指标[膀胱容量、膀胱顺应性(BC)、残余尿量(PVR)]、尿动力学指标[膀胱逼尿肌压力(BDP)、最大尿道压力(MUP)、Qmax尿流率],并采用SF-36量表调查患者生活质量。结果:护理后,A+P组患者膀胱容量、BC均高于A组(P<0.05),PVR低于A组(P<0.05);A+P组患者BDP、MUP、Qmax尿流率均高于A组(P<0.05)。护理后,A+P组患者SF-36量表各维度评分均高于A组(P<0.05)。结论:术后予以膀胱癌患者膀胱功能重建护理能有效提高患者膀胱功能,改善尿动力学,提高其生活质量。Objective:To study the implementation method of bladder function reconstruction intervention and its application value in postoperative nursing of patients with bladder cancer.Methods:108 cases of patients undergoing bladder cancer surgery in a hospital were randomly divided into group A(54 cases)and group A+P(54 cases).Group A carried out routine nursing,and group A+P carried out bladder function reconstruction nursing on the basis of group A(mainly including active prevention of postoperative complications,improvement of bladder compliance,bladder function rehabilitation exercise,etc).Before and after nursing,bladder function indexes[bladder volume,bladder compliance(BC),residual urine volume(PVR)],and urodynamic indexes[bladder detrusor pressure(BDP),maximum urethral pressure(MUP),Qmax urine flow rate]were measured,and sF-36 scale was used to investigate the quality of life of patients.Results:After nursing,bladder volume and BC in A+P group were higher than those in A group(P<0.05),and PVR was lower than that in A group(P<0.05).BDP,MUP and Qmax urine flow rate in A+P group were higher than those in A group(P<0.05).After nursing,the scores of all dimensions of SF-36 scale in A+P group were higher than those in A group(P<0.05).Conclusion:Postoperative bladder function reconstruction nursing for patients with bladder cancer can effectively improve the bladder function,improve urodynamics and improve the quality of life.
