血清PCT、BNP水平与脓毒症患者肝损伤相关指标的相关性分析

摘　　要：目的 分析血清降钙素原(procalcitonin, PCT)、B型利钠肽(B-type natriuetic peptide, BNP)水平与脓毒症患者肝损伤相关指标的相关性。方法 选取2019年6月~2021年6月我院收治的151例脓毒症患者作为研究对象,根据患者是否存在肝损伤分为脓毒症组(n=48)和脓毒症肝损伤组(n=103),并根据肝损伤程度将脓毒症肝损伤组患者分为轻度肝损伤组(n=26)、中度肝损伤组(n=50)和重度肝损伤组(n=27)。于患者入院时测定PCT、BNP水平及肝功能指标:谷草转氨酶(AST)、谷丙转氨酶(ALT)、总胆红素(TBIL)、直接胆红素(DBIL)、白蛋白(ALB)。比较脓毒症组、脓毒症肝损伤组血清PCT、BNP水平及肝功能指标。比较轻度肝损伤组、中度肝损伤组、重度肝损伤组血清PCT、BNP水平及肝功能指标。分析血清PCT、BNP水平与AST、ALT、TBIL、DBIL、ALB的相关性。结果 脓毒症组血清PCT、BNP、AST、ALT、TBIL、DBIL水平显著低于脓毒症肝损伤组,ALB水平显著高于脓毒症肝损伤组(P<0.05)。脓毒症肝损伤组3组血清PCT、BNP、AST、ALT、TBIL、DBIL水平比较,轻度<中度<重度,差异均有统计学意义(P<0.05);3组血清ALB水平比较,轻度>中度>重度,差异有统计学意义(P<0.05)。经相关性分析,血清PCT、BNP水平与AST、ALT、TBIL、DBIL呈明显正相关,与ALB呈明显负相关,差异有统计学意义(P<0.05)。结论 血清PCT、BNP水平与脓毒症肝损伤及肝功能指标具有明显相关性。Objective To analyze the correlation between serum procalcitonin(PCT),B-type natriuretic peptide(BNP)and liver injury in patients with sepsis.Methods A total of 151 patients with sepsis treated in our hospital from June 2019 to June 2021 were selected,and the patients were divided into sepsis group(n=48)and sepsis liver injury group(n=103),according to the degree of liver injury.The patients in sepsis liver injury group were divided into mild liver injury group(n=26),moderate liver injury group(n=50)and severe liver injury group(n=27).PCT,BNP level and liver function indexes(AST,ALT,TBIL,DBIL and ALB)were measured at admission.The serum PCT,BNP levels and liver function indexes of sepsis group and sepsis liver injury group were compared.The serum PCT,BNP levels and liver function indexes of mild liver injury group,moderate liver injury group and severe liver injury group were compared.The correlation between serum PCT,BNP level and AST,ALT,TBIL,DBIL and ALB was analyzed.Results The levels of serum PCT,BNP,AST,ALT,TBIL and DBIL in sepsis group were significantly lower than those in sepsis liver injury group,and the level of ALB was significantly higher than that in sepsis liver injury group(P<0.05).The levels of serum PCT,BNP,AST,ALT,TBIL and DBIL in the three groups were mildmoderate>severe,and the difference was statistically significant(P<0.05).By correlation analysis,serum PCT,BNP level was significantly positively correlated with AST,ALT,TBIL and DBIL,and negatively correlated with ALB(P<0.05).Conclusion The levels of serum PCT and BNP are significantly correlated with liver injury and liver function in sepsis.
