健脾益肾综合针法对围绝经期综合征患者生活质量的影响

摘　　要：目的探讨健脾益肾综合针法对围绝经期综合征患者生活质量的影响。方法180例围绝经期综合征患者随机分为A组、B组和C组,每组各60例。采用单盲法,研究结束时共172例患者完成临床观察,共脱落8例,其中A组56例、B组59例、C组57例。A组采用温针治疗,B组采用耳针治疗,C组采取健脾益肾综合针,即A组+B组的综合治疗方法。采用更年期生活质量评定量表(MRS)、改良Kupperman量表(KPS)评定治疗前后两组患者的生活质量及躯体症状,并进行评分变化的比较。结果A、B、C三组围绝经期综合征患者的MRS评分及改良KPS评分均随着治疗时间的增加而明显下降(P<0.05),且C组下降最为明显(P<0.05),A组次之,B组下降幅度最小;同时,MRS评分、改良KPS评分的分组效应和时间效应之间的交互作用具有统计学意义(P<0.05),表明不同疗法的疗效具有差异。治疗结束后1个月随访,MRS评分、改良KPS评分均较治疗刚结束时有所上升,但其他两组的上升程度明显高于C组,差异有统计学意义(P<0.05)。结论健脾益肾综合针法能够显著改善围绝经期综合征患者的躯体症状和生活质量,近期及远期疗效均佳。Objective To explore the effect of comprehensive acupuncture for strengthening the spleen and nourishing the kidney on the quality of life of patients with perimenopausal syndrome.Methods 180 patients with perimenopausal syndrome were randomly divided into group A,group B and group C,with 60 cases in each group.Using single blind method,172 patients completed clinical observation at the end of the study,and 8 cases fell off,including 56 cases in group A,59 cases in group B and 57 cases in group C.Group A was treated with warm needle acupuncture,group B was treated with auricular acupuncture,and group C was treated with comprehensive acupuncture for strengthening the spleen and nourishing the kidney,wthich is the comprehensive treatment method of group A and C.Menopausal rating scale(MRS)and modified Kupperman scale(KPS)were used to evaluate the quality of life and physical symptoms of the two groups before and after treatment,and the score changes of scores were compared.Results MRS score and modified KPS score of patients with perimenopausal syndrome in group A,B and C decreased significantly with the increase of treatment time(P<0.05),and the decrease was most significant in group C(P<0.05),followed by group A and group B.At the same time,the interaction between grouping effect and time effect of MRS score and modified KPS score was statistically significant(P<0.05),indicating that the curative effects of different therapies were different.During the follow-up one month after the treatment,MRS score and modified KPS score increased compared with that at the end of the treatment,but the increase degree of the other two groups was significantly higher than that of group C(P<0.05).Conclusion Comprehensive acupuncture for strengthening the spleen and nourishing the kidney can significantly improve the symptoms and quality of life of patients with perimenopausal syndrome,and both short-term and long-term effects exist.
