益气健脾利水方治疗慢性心力衰竭疗效观察及对血浆脑钠肽水平的影响

摘　　要：目的:观察中药益气健脾利水方治疗慢性心力衰竭(CHF)的效果及对患者血浆脑钠肽(BNP)水平的影响。方法:纳入CHF患者76例,按随机数字表法分为观察组和对照组各38例。2组均给予基础治疗,对照组给予扩张血管、强心利尿、β受体阻滞剂等常规西药治疗,观察组在对照组的基础上加用益气健脾利水方治疗。观察2组临床疗效、血浆BNP水平、6 min步行距离及心功能[心脏每搏量(SV)、左心室射血分数(LVEF)]改善情况。结果:观察组临床总有效率为89.47%,高于对照组的68.42%,差异有统计学意义(P<0.05)。治疗前,2组血浆BNP水平、6 min步行距离比较,差异无统计学意义(P>0.05)。治疗后,2组血浆BNP水平均较治疗前明显降低(P<0.05),6 min步行距离均较治疗前显著增加(P<0.05);且观察组BNP水平低于对照组(P<0.05),6 min步行距离长于对照组(P<0.05)。治疗前,2组SV、LVEF水平比较,差异无统计学意义(P>0.05);治疗后,2组SV、LVEF水平均较治疗前明显升高(P<0.05),且观察组2项指标均高于对照组(P<0.05)。结论:采用中药益气健脾利水方治疗CHF疗效显著,可改善心功能及血浆BNP水平,延长6 min步行距离。Objective:To observe the effects of Yiqi Jianpi Lishui prescription on chronic heart failure(CHF)and brain natriuretic peptide(BNP)level in plasma.Methods:A total of 76 cases of CHF patients were included and divided into the observation group and the control group according to the random number table method,with 38 cases in each group.Both groups were given basic treatment;the control group was givenβblockers and other routine western medicine treatments to dilate blood vessels and strengthen heart and promote diuresis;the observation group was additionally treated with Yiqi Jianpi Lishui prescription based on the treatment of the control group.Clinical effects,levels of BNP in plasma,6-minute walking distance and the improvement in heart function[stroke volume(SV)、left ventricular ejection fractions(LVEF)]were observed in the two groups.Results:The total effective rate in clinic was 89.47%in the observation group,higher than that of 68.42%in the control group,the difference being significant(P<0.05).Before treatment,there was no significant difference being found in the comparison of BNP levels in plasma and 6-minute walking distance between the two groups(P>0.05).After treatment,BNP levels in plasma in the two groups were significantly decreased when compared with those before treatment(P<0.05),and 6-minute walking distances were significantly increased(P<0.05);BNP level in the observation group was lower than that in the control group(P<0.05),and 6-minute walking distance was higher(P<0.05).Before treatment,there was no significant difference being found in the comparison of levels of SV and LVEF between the two groups(P>0.05).After treatment,levels of SV and LVEF in the two groups were significantly increased when compared with those before treatment(P<0.05),and the two indexes in the observation group were higher than those in the control group(P<0.05).Conclusion:Yiqi Jianpi Lishui prescription has a significant curative effect in the treatment of CHF and can improve heart function,BNP and prolong 6-minu
