伤科接骨片联合锁定钢板内固定术治疗胫骨骨折临床研究

摘　　要：目的:观察伤科接骨片联合锁定钢板内固定术治疗胫骨骨折的临床疗效。方法:选取胫骨骨折患者82例,按照随机数字表法分为对照组和观察组各41例。2组均给予锁定钢板内固定术治疗,对照组术后给予常规治疗,观察组在对照组的基础上加用伤科接骨片治疗。2组均连续治疗12周。比较2组手术时间、术中出血量、住院时间以及治疗后疼痛视觉模拟评分法(VAS)评分、骨折愈合率及临床疗效。结果:观察组总有效率为97.56%,对照组为80.49%,2组比较,差异有统计学意义(P<0.05)。2组手术时间、术中出血量、住院时间比较,差异均无统计学意义(P>0.05)。术后24 h,2组VAS评分比较,差异无统计学意义(P>0.05);治疗6、12周,2组VAS评分均较术后24 h减少(P<0.05),且观察组同时间点VAS评分均低于对照组(P<0.05)。治疗12周,观察组骨折愈合率为58.54%,对照组为34.15%,2组比较,差异有统计学意义(P<0.05)。结论:伤科接骨片联合锁定钢板内固定术治疗胫骨骨折的疗效显著,并可有效减轻患者术后疼痛,促进骨折愈合。Objective:To observe the clinical effect of Shangke Jiegu pills combined with internal fixation with locking plate for tibial fracture.Methods:A total of 82 cases of patients with tibial fracture were selected and divided into the observation group and the control group according to random number table method,with 41 cases in each group.Both groups were given internal fixation with locking plate.After the fixation,the control group was given routine treatment,and the observation group was treated with Shangke Jiegu pills and routine treatment.Both groups were treated for 12 weeks.Operation time,intraoperative blood loss and hospitalization time were compared between the two groups.The scores of Visual Analogue Scale(VAS)for pain after treatment,fracture healing rates and clinical effects were compared between the two groups.Results:The total effective rate was 97.56%in the observation group and 80.49%in the control group(P<0.05),the difference being significant(P<0.05).There was no significant difference being found in comparisons of operation time,intraoperative blood loss and hospitalization time between the two groups(P>0.05).There was no significant difference being found in the comparison of VAS scores at the 24 hours after fixation between the two groups(P>0.05).After 6 and12 weeks of treatment,the VAS scores at different time points in the two groups were decreased when compared with24 hours after fixation(P<0.05),and the score in the observation group was lower than that in the control group(P<0.05).After 12 weeks of treatment,the fracture healing rate was 58.54%in the observation group and 34.15%in the control group,the difference being significant(P<0.05).Conclusion:Shangke Jiegu pills combined with internal fixation with locking plate has a significant curative effect in the treatment of tibial fracture,which can effectively relieve postoperative pain and promote fracture healing.
