摘 要:目的探讨核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)、谷胱甘肽S-转移酶(GSTs)、Toll样受体4(TLR4)基因多态性与慢性阻塞性肺疾病(COPD)发病的关系。方法选取该院2017年1月至2021年1月收治的131例COPD患者为观察组,另选取同期健康体检者120例为对照组。采用限制性片段长度多态性聚合酶链反应(PCR-RFLP)检测NLRP3、GSTs、TLR4基因多态性,Hardy-Weinberg平衡检验基因型分布遗传平衡,logistic回归方程分析影响COPD发生的危险因素。结果两组性别、年龄、饮酒比例、收缩压、舒张压、空腹血糖、总胆固醇、甘油三酯、尿酸等比较差异均无统计学意义(P>0.05),观察组吸烟比例明显高于对照组(P<0.05)。单位点分析结果显示,NLRP3 rs3806265位点、TLR4 rs2737190、rs10759932位点与COPD发病有关联(P<0.05);吸烟、NLRP3 rs3806265位点、TLR4 rs2737190、rs10759932位点是COPD发病的独立影响因素(P<0.05)。结论NLRP3 rs3806265位点、TLR4 rs2737190、rs10759932位点基因多态性影响COPD易感性。Objective To investigate the relationship between nucleotide binding oligomerization domain like receptor protein 3(NLRP3),glutathione S-transferases(GSTs)and toll-like receptors 4(TLR4)gene polymorphisms with the onset of chronic obstructive pulmonary disease(COPD).Methods A total of 131 patients with COPD treated in this hospital from January 2017 to January 2021 were selected as the observation group,and 120 subjects undergoing healthy physical examination during the same period were selected as the control group.The polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)was used to detect NLRP3,GSTs and TLR4 gene polymorphisms.The Hardy-Weinberg equilibrium was used to test the genetic balance of genotype distribution.The logistic regression equation was used to analyze the risk factors affecting the onset of COPD.Results No statistically significant differences were found in the gender,age,alcohol drinking,systolic blood pressure,diastolic blood pressure,fasting blood glucose,total cholesterol,triglyceride and uric acid between the two groups(P>0.05),while the proportion of the persons with smoking in the observation group was significantly more than that in the control group(P<0.05).The unit point analysis results showed that NLRP3 rs3806265,TLR4 rs2737190 and rs10759932 loci had the relation with the COPD onset(P<0.05);smoking,NLRP3 rs3806265 locus,TLR4 rs2737190 locus and TLR4 rs10759932 locus were the independent influencing factors of the COPD onset(P<0.05).Conclusion The gene polymorphisms at NLRP3(rs3806265)locus,TLR4 rs2737190 locus and TLR4 rs10759932 locus affect COPD susceptibility.
