金水六君煎加减治疗老年气虚痰阻型慢性阻塞性肺疾病急性期临床研究

摘　　要：目的:观察金水六君煎加减对气虚痰阻型慢性阻塞性肺疾病(COPD)急性期老年患者的疗效及对肺功能、炎症因子的影响。方法:将85例气虚痰阻型COPD急性期患者随机分为2组。对照组45例采用布地奈德+异丙托溴铵治疗;观察组40例采用金水六君煎加减治疗,持续治疗8周。比较2组临床疗效及治疗前后血清降钙素原(PCT)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平及第1秒用力呼气容积(FEV_(1))、第1秒用力呼气容积占预计百分比(FEV_(1)%)、用力肺活量(FVC)。结果:与同组治疗前比较,治疗后2组血清PCT、IL-6、IL-8水平均降低(P<0.05);与对照组治疗后比较,观察组治疗后血清PCT、IL-6、IL-8水平均较低(P<0.05)。与同组治疗前比较,治疗后2组FEV_(1)、FEV_(1)%、FVC水平均升高(P<0.05);与对照组治疗后比较,观察组治疗后FEV_(1)、FEV_(1)%、FVC水平均较高(P<0.05)。对照组总有效率75.56%,低于观察组92.50%(P<0.05)。结论:金水六君煎加减治疗气虚痰阻型COPD急性期老年患者疗效显著,可改善肺功能,降低炎症因子水平。Objective:To observe the curative effect of modified Jinshui Liujun decoction on senile patients with chronic obstructive pulmonary disease(COPD)at acute stage of qi deficiency and phlegm obstruction type,and its effects on lung function and inflammatory factors.Methods:A total of 85 COPD patients at acute stage of qi deficiency and phlegm obstruction type were randomly divided into two groups.Forty five cases in the control group were treated with budesonide and ipratropium bromide,and forty cases in the observation group were treated with modified Jinshui Liujun decoction.The treatment lasted for eight weeks.The clinical effect,serum levels of procalcitonin(PCT),interleukin-6(IL-6),interleukin-8(IL-8),forced expiratory volume in one second(FEV_(1)),percentage of forced expiratory volume in one second(FEV_(1)%)and forced vital capacity(FVC)were compared between the two groups before and after treatment.Results:Compared with those in the same group before treatment,the levels of serum PCT,IL-6 and IL-8 in both groups were decreased after treatment(P<0.05).Compared with those in the control group after treatment,the serum levels of PCT,IL-6 and IL-8 in the observation group were lower after treatment(P<0.05).Compared with those in the same group before treatment,the levels of FEV_(1),FEV_(1)%and FVC in both groups were increased after treatment(P<0.05).Compared with those in the control group,the levels of FEV_(1),FEV_(1)%and FVC in the observation group were higher after treatment(P<0.05).The total effective rate was 75.56%in the control group,lower than that of 92.50%in the observation group(P<0.05).Conclusion:Modified Jinshui Liujun decoction has a significant therapeutic effect on elderly COPD patients at acute stage of qi deficiency and phlegm obstruction type,which can improve lung function and reduce the levels of inflammatory factors.
