摘 要:目的探讨染色体上同时携带碳青霉烯酶基因bla_(NDM-1)和bla_(OXA-23)的鲍曼不动杆菌的分布特点,深入分析位于染色体上的bla_(NDM-1)和bla_(OXA-23)所在的遗传学环境。方法,通过NCBI库检索世界范围内所有染色体上同时携带bla_(NDM-1)和bla_(OXA-23)的鲍曼不动杆菌,通过生物信息学分析染色体中携带bla_(NDM-1)和bla_(OXA-23)的移动元件的遗传学特征。结果共收集到18例染色体上同时携带bla_(NDM-1)和bla_(OXA-23)的鲍曼不动杆菌,所有菌株都分离自人体,分离年份集中在近几年,主要分布在印度(83.3%,15/18),标本来源主要为呼吸道和血液标本,bla_(NDM-1)主要位于完整的Tn125结构中,bla_(OXA-23)3全部位于完整的Tn2006结构中,Tn2006可形成多个拷贝位于染色体中。结论近几年临床分离的位于染色体上同时携带bla_(NDM-1)和bla_(OXA-23)3的鲍曼不动杆菌数量明显增加,且都位于复合转座子中,bla_(OXA-23)3可形成多个拷贝,其未来的传播风险应引起临床重视。Objective To investigate the distribution characteristics of Acinetobacter baumannii co-carrying carbapenase genes bla_(NDM-1) and bla_(OXA-23) in their chromosome The genetic environment of bla_(NDM-1) and bla_(OXA-23) located within corresponding chromosomes was performed.Methods All Acinetobacter baumanni strains with bla_(NDM-1) and bla_(OXA-23) on their chromosomes were worldwide searched by NCBI database,and the genetic characteristics of mobile elements carrying bla_(NDM-1) and bla_(OXA-23) were analyzed by bioinformatics methods Results Atotal of 18co-carrying bla_(NDM-1) and bla_(OXA-23)in the chromosome of Acinetobacter baumannii were collected All the strains were isolated from human samples in recent years,mainly distributed in India(83.3%,15/18).These samples were mainly from respiratory tract and blood.Most of the bla_(NDM-1) were located in the intact structure of Tn125,and allof the bla_(OXA-23) were located in the intact structureof Tn2006 whichcould formed multiple copieson the chromosomes.Conclusion Thenumberof clinically relevant Acinetobacter baumannii co-carrying bla_(NDM-1) and bla_(OXA-23) in their chromosomes has increased significantly in recent years Both of the bla_(NDM-1) and bla_(OXA-23) were carried by composite transposons,and bla_(OXA-23) could form multiple copies Clinical attention should bepaid tothe risk of future transmission.
