外周血B淋巴细胞中Blimp-1蛋白表达在原发性免疫性血小板减少性紫癜患者治疗中的应用

摘　　要：目的探讨原发性免疫性血小板减少性紫癜(ITP)患者治疗前后外周血B淋巴细胞中Blimp-1表达与病情及预后的相关性。方法将75例ITP患者分为激素治疗有效组和无效组:选择健康体检者30例为对照组,检测各组治疗前,治疗后4周、3个月、6个月外周血CD19^(+)细胞、Blimp-1^(+)CD19^(+)细胞、Blimp-1^(+)B1细胞、PA IgG与PLT水平变化进行比较。结果初治75例ITP外周血CD19^(+)细胞、Blimp-1^(+)CD19^(+)细胞、Blimp-1 B1细胞、PA IgG水平明显高于对照组,治疗后,激素治疗有效组外周血Blimp-1^(+)CD19^(+)细胞、Blimp-1^(+)B1细胞、PA IgG水平较治疗前明显下降差异均有统计学意义(P<0.05):而激素治疗无效组则无明显变化。结论初治ITP患者外周血B淋巴细胞异常高表达,外周血B淋巴细胞中Blimp-1高表达可能与ITP发病有关,治疗前后外周血B细胞中Blimp-1表达变化与病情和预后的存在一定的相关性。Objective To investigate the correlation between Blimp-1 expression in peripheral blood B lymphocytes of patients with idiopathic thrombocytopenic purpura(ITP)before and after treatment and their disease condition and prognosis.Methods Seventy five patients with ITP were divided into effective group and ineffective group,and thirty cases were collected into the control group.The changes of peripheral blood CD19^(+)cells,Blimp-1^(+)CD19^(+)cells,Blimp-1^(+)B1 cells,PA IgG and PLT were compared between before and at4 w,3 m and 6 m after treatment.Results The levels of CD19^(+)cells,Blimp-1^(+)CD19^(+)cells.Blimp-1^(+)B1 cells and PA IgG in peripheral blood of the patients with ITP were significantly higher than those in the control group.After treatment,the levels of Blimp-1^(+)CD19^(+) cells,Blimp-1^(+)B1 cells and PA IgG in peripheral blood of the effective hormone treatment group were significantly lower than those before treatment,with the differences statistically significant(P<0.05).There was no significant change in the ineffective hormone treatment group.Conclusion The high expression of Blimp-1 in peripheral blood B lymphocytes may be related to the pathogenesis of ITP.There is a certain correlation between the expression of Blimp-1 in peripheral blood B cells before and after treatment and the condition and prognosis.
