摘　　要：目的探讨他达拉非联合加速康复外科干预治疗经尿道钬激光前列腺剜除术(HoLEP)后勃起功能障碍(ED)患者的临床效果。方法选取2018年1月至2020年12月南通大学附属海安医院泌尿外科收治的108例HoLEP后ED患者作为研究对象。随机分为观察组与对照组,各54例。对照组采用加速康复外科干预治疗,观察组采用他达拉非联合加速康复外科干预治疗。比较两组治疗前后的国际勃起功能指数-5(IIEF-5)评分、勃起硬度评分法(EHS)评分、射精功能量表(CIPE)评分、阴道内射精潜伏期(IELT)、性生活日志(SEP)评分和性生活质量量表(QISGT)评分,统计观察组药物不良反应的发生情况。结果治疗8周时,两组IIEF-5评分、EHS评分、CIPE评分、IELT、SEP评分和QISGT评分均较治疗前升高,且观察组的升高幅度高于对照组,差异具有统计学意义(P<0.05)。治疗24周时,两组IIEF-5评分、EHS评分、CIPE评分、IELT、SEP评分和QISGT评分均较治疗前、治疗8周时升高,且观察组的升高幅度高于对照组,差异具有统计学意义(P<0.05)。观察组随访24周仅发生4例轻微的药物不良反应。结论他达拉非联合加速康复外科干预治疗HoLEP后ED患者,能有效改善阴茎勃起和射精功能,提高性生活质量。Objective To investigate the clinical effect of Tadalafil combined with accelerated rehabilitation surgical intervention on erectile dysfunction(ED) after transurethral holmium laser enucleation of prostate(HoLEP). Methods 108 patients with ED after HoLEP who were admitted to the Department of Urology, Hai′an Hospital of Nantong University from January 2018 to December 2020 were selected as the research objects, and were randomly divided into observation group and control group, 54 cases in each group. The control group was treated with accelerated rehabilitation surgical intervention, and the observation group was treated with Tadalafil combined with accelerated rehabilitation surgical intervention. The international erectile function index-5(IIEF-5), erectile hardness score(EHS), Chinese index of sexual funtion for premature ejaculation(CIPE), intra-vaginal ejaculation latency time(IELT), sexual life log(SEP) and sexual life quality scale(QISGT) were compared between the two groups before and after treatment. The incidence of adverse drug reactions in observation group was statistically analyzed. Results After 8 weeks of treatment, IIEF-5, EHS, CIPE, IELT, SEP and QISGT in both groups were higher than those before treatment, and the increase range in observation group was higher than that in control group(P<0.05). After 24 weeks of treatment, IIEF-5, EHS, CIPE, IELT, SEP and QISGT in both groups were higher than those before and 8 weeks of treatment, and the increase range in observation group was higher than that in control group(P<0.05). In the observation group, only 4 cases of mild adverse drug reactions occurred after 24 weeks of follow-up. Conclusions Tadalafil combined with accelerated rehabilitation surgical intervention in the treatment of ED after HoLEP can effectively improve penile erection and ejaculation function, and improve quality of sexual life.
