禁欲时间对男性精子质量的影响

摘　　要：目的研究禁欲时间对男性精子质量的影响,探讨快速改善受精日精子质量的办法,建立提高精液分析准确性的有效措施。方法选取2019年4月至9月北部战区总医院诊治的1479例男性患者作为研究对象。均进行精液常规分析及精子染色质结构分析,根据禁欲时间依次排列影响因素,并进一步进行分组比较。结果随着禁欲时间的延长,精液体积、精子总数、精子浓度、非前向运动精子率及精子DNA碎片率(DFI)均有不同程度的增高。与短期禁欲组比较,WHO推荐禁欲组的精液体积、精子总数和DFI明显增高,差异具有统计学意义(P<0.05)。与长期禁欲组比较,WHO推荐禁欲组的精液体积、精子总数、精子浓度、非前向运动精子率和DFI明显降低,差异具有统计学意义(P<0.05)。结论禁欲时间会对精液各项参数产生显著影响。为了获得稳定的精液分析结果,建议禁欲时间为5~6d。为了获得最小的DFI、最多的前向活动精子及更好的精子质量,建议禁欲时间为1~2d。Objective To study the effect of abstinence time on male sperm quality, and explore ways to quickly improve sperm quality on fertilization day, in order to establish effective measures to improve the accuracy of semen analysis. Methods 1,479 male patients treated in Northern Theater General Hospital from April to September 2019 were selected as the research objects. Semen routine analysis and sperm chromatin structure analysis were performed of them, and the influencing factors were arranged according to the abstinence time, and further grouped to compare. Results With the extension of abstinence time, semen volume, total sperm count, sperm concentration, non-forward motility sperm percentage and sperm DNA fragmentation index(DFI) were increased in varying degrees. Compared with the short-term abstinence group, the semen volume, total sperm count and DFI of the WHO recommends abstinence group were significantly higher(P<0.05). Compared with the long-term abstinence group, semen volume, total sperm count, sperm concentration, non-forward motility sperm percentage and DFI of the WHO recommends abstinence group were significantly lower(P<0.05). Conclusions Abstinence time has a significant impact on semen parameters. In order to obtain stable semen analysis results, it is suggested that the abstinence time is 5-6 d. In order to obtain the minimum DFI, the maximum forward motility sperm and better sperm quality, it is suggested that the abstinence time is 1-2 d.
