血液分析自动审核的临床应用

摘　　要：目的对血液分析自动审核规则应用于临床血液检验进行验证,并分析自动审核规则的临床应用价值。方法选取宁波市第一医院检验科2020年3月-2021年9月的EDTA-K2抗凝血常规样本共363799例,其中规则验证组8656例;临床应用组355143例。应用SysmexXN系列全自动血液分析仪检测血常规,同时制备血涂片、染色、镜检,将临床信息、设备参数、检测结果和镜检结果汇总。验证结束后,分别计算真阳性、假阳性、真阴性、假阴性、自动审核率和通过正确率。应用自动审核规则自动审核血常规结果验证漏检率,得出自动审核率、实验室周转时间(TAT)和.30 min内报告提高百分比。结果自动审核验证组中7115例真阳性率为15.99%(1138/7115)、假阳性率为6.00%(427/7115)、真阴性率为75.01%(5337/7115)、假阴性率为2.99%(213/7115),自动审核率为78.00%、通过正确率为96.16%,1541例Deltacheck真阳性率为46.00%、假阳性率为8.96%、真阴性率为45.04%、假阴性率为0.00%。血液分析应用自动审核规则前后平均TAT分别为38.00 min、31.29 min,缩短了6.71 min,30 min内发放报告的百分率由55.93%增加到61.79%,自动审核通过率为78.77%。结论血液分析应用自动审核规则后,保证了血常规检验质量同时能够明显缩短TAT,提升报告发放的及时性,提高实验室工作效率。Objective The application of automatic verification rules for blood analysis to clinical blood tests was verified and its clinical application value was analyzed. Methods A total of 363 799 EDTA-K_(2) anticoagulant blood samples were collected from the Department of Laboratory Medicine,the First Hospital of Ningbo from March 2020 to September 2021,of which 8 656 cases were selected in the auto verification validation group,and 355 143 cases in the clinical application trial group;Sysmex XN series automatic blood analyzer was used to detect blood routinely,prepare blood smears,stains,and microscopy for each sample,and summarize the clinical information,equipment parameters,test results,and microscopy results. When the auto verification rules were undergone validation,true positive,false positive,true negative,false negative,automatic review rate,and passing accuracy rate were calculated. The auto verification rules were applied to audit blood routine results and missing rates were validated. Automatic verification rules were applied to verify the missed rate of blood routine results,and the automatic audit rate,laboratory turnaround time( TAT),and percentage of reported improvement within 30 min were obtained. Results In the automatic verification group,the true positive rate was 15. 99%( 1 138/7 115),the false positive rate was 6. 00%( 427/7 115),the true negative rate was 75. 01%( 5 337/7 115) and the false negative rate was 2. 99%( 213/7 115). The automatic examination rate was 78. 00%,the correction rate was 96. 16%. The true positive rate,false positive rate,true negative rate and false negative rate of 1 541 Delta check cases was respectively 46. 00%,8. 96%,45. 04% and 0. 00%. The average TAT before and after the application of automatic verification rule was 38. 00 min and 31. 29 min respectively,which was shortened by 6. 71 min.
