摘  要:目的:探讨足三里和肾腧穴位埋线(acupoint catgut embedding,ACE)通过核因子κB(nuclear factor-κB,NF-κB)信号通路减轻大鼠类风湿关节炎(rheumatoid arthritis,RA)的机制。方法:采用雌性SD大鼠,构建大鼠RA模型,分为模型组、来氟米特(leflunomide,LEF)组、ACE组和ACE+LEF组,另设对照组,每组6只。关节炎指数评估关节炎的严重程度;HE染色观察大鼠滑膜组织病理学变化;ELISA法检测各组大鼠血清中白细胞介素6(interleukin-6,IL-6)和IL-8含量的变化;免疫组化法检测滑膜组织NF-κB p65的表达;Western blot检测大鼠滑膜组织p-p65蛋白水平。结果:与对照组相比,模型组大鼠表现出滑膜增生,滑膜组织中炎症细胞浸润,关节软骨侵蚀;ACE及LEF治疗的RA大鼠滑膜细胞增生和炎症细胞浸润均有所缓解。与对照组相比,模型组大鼠关节炎指数、血清炎症因子IL-6和IL-8水平及滑膜组织NF-κB p65和p-p65蛋白水平均显著升高(P<0.01);与模型组相比,LEF和ACE单独或联合治疗后,大鼠关节炎指数、血清炎症因子IL-6和IL-8水平及滑膜组织NF-κB p65和p-p65蛋白水平均显著降低(P<0.05),其中ACE+LEF组降低最显著。结论:ACE对于RA大鼠的炎症反应有明显的抑制作用,其机制可能与抑制NF-κB信号通路中p65蛋白的表达及磷酸化来抑制关节炎症反应及损伤有关。AIM:To investigate the effect of Zusanli and Shenshu acupoint catgut embedding(ACE)on rat rheumatoid arthritis(RA).METHODS:Female SD rats were used to construct rat RA model. The rats were divided into control group,model group,leflunomide(LEF)group,ACE group,and ACE+LEF group,with 6 rats in each group. The severity of arthritis was evaluated by the arthritis index. Histopathological changes of synovial tissues were observed by HE staining. Serum interleukin-6(IL-6)and IL-8 levels were detected by ELISA. The expression of NF-κB p65 in synovial tissues was detected by immunohistochemistry. The protein level of p-p65 in synovial tissues of the rats was detected by Western blot.RESULTS:Synovial hyperplasia,infiltration of inflammatory cells in synovial tissue and erosion of articular cartilage were observed in model group. The synovial hyperplasia and inflammatory cell infiltration were alleviated in RA rats after treatment with ACE and LEF. Compared with control group,arthritis index,serum inflammatory factor IL-6and IL-8 levels,and synovial NF-κB p65 and p-p65 protein levels were significantly increased in model group(P<0. 01).Compared with model group,arthritis index,serum inflammatory factor IL-6 and IL-8 levels,and synovial NF-κB p65 and p-p65 protein levels were significantly decreased after treatment with LEF and ACE,either alone or in combination(P<0. 05),and the combined treatment was significantly better than the single treatment.CONCLUSION:ACE inhibits the inflammatory response and injury of arthritis by inhibiting NF-κB signaling pathway.
