“一针法”胰肠吻合在腹腔镜胰十二指肠切除术中安全性评价

摘　　要：目的探讨“一针法”胰肠吻合术在腹腔镜胰十二指肠切除术(LPD)中的安全性和有效性。方法回顾性分析2018年1月至2021年1月100例行LPD患者的临床资料,根据不同胰肠吻合方式分为传统组(n=56例)和一针法组(n=44例),其中传统组采用胰管空肠黏膜吻合方式,一针法组采用一针法胰肠吻合方式。使用SPSS25.0处理数据,围手术期指标以(x±s)表示,采用独立样本t检验;胰腺质地、术后病理类型及并发症发生率采用卡方检验或秩和检验。P<0.05为差异有统计学意义。结果一针法组的手术时间和胰肠吻合时间均短于传统组(P<0.05)。两组患者术后胰瘘、胆瘘及胃排空障碍等发生率比较差异均无统计学意义(P>0.05)。结论“一针法”胰肠吻合可有效缩短术中吻合时间,缝合难度不受胰管大小限制,是LPD中一种安全可行的吻合方式。Objective To investigate the safety and efficacy of“single-stitch”pancreaticejejunostomy in laparoscopic pancreaticoduodenectomy(LPD).Methods The clinical data of 100 patients who underwent LPD from January 2018 to January 2021 were retrospectively analyzed.According to the anastomosis method,they were divided into traditional group(n=56 cases)and single-stitch group(n=44 eases),in which the traditional group adopted pancreaticojejunostony and the one-needle group adopted single-stich pancreaticojejunostomy.SPSS 25.0 was used for data processing.Perioperative indieators were expressed as(x±s)and independent sample t test was used.Pancreatic texture,postoperative pathological types and incidence of complications were tested by Chi-square test or Rank-sum test.P<0.05 was considered statistically signifcant.Results The operation time and panereat icojejunostomy time in the single-stitch anastomosis group were shorter than those in the traditional group(P<0.05).There were no significant differences in the incidence of posloperative pancreatice fistula,bile leakage and gastric emptying disorders between 2 groups(P>0.05).Conclusion"Single-sitch"pancreaticojejunostomy can effectively shorten the intraoperative anastomosis time,and the dificulty of suture is not limited by the size of the pancreatice duct,so it is a safe and feasible anastomosis method in LPD.
