摘  要:目的 调查海南省农村成人高尿酸血症患病现况和影响因素。方法 采用多阶段分层整群随机抽样的方法,于2018—2020年选取海南省农村18岁及以上1 120人为研究对象,对高尿酸血症患病现况及危险因素进行分析。结果 海南省农村成人高尿酸血症的粗患病率为23.6%,标化患病率为31.9%。男性高尿酸血症的粗患病率为50.1%,标化患病率57.6%,青年男性(18~44岁)的患病率最高,为53.5%;女性高尿酸血症的粗患病率为9.4%,标化患病率9.1%。高尿酸血症人群中26.1%合并糖尿病,39.0%合并高血压,55.3%合并血脂异常,57.6%合并超重和肥胖,47.7%合并代谢综合征,均明显高于非高尿酸血症人群。Logistic回归分析显示体质指数(BMI)高、低密度脂蛋白胆固醇(LDL)高是我省农村成年男性高尿酸血症的危险因素,空腹血糖(FBG)高是保护性因素;年龄、BMI、LDL高是我省农村成年女性高尿酸血症的危险因素,高密度脂蛋白胆固醇高是保护性因素。男性尿酸(UA)的预测模型为UA=351.443+3.867×BMI-9.322×FBG+3.197×糖负荷后2小时血糖(2hPBG);女性尿酸的预测模型UA=286.236+4.195×BMI-17.024×糖化血红蛋白+18.028×高血压+2.966×2hPBG+13.163×血脂异常。结论 海南省农村成人高尿酸血症的防控形势十分严峻,尤其是青年男性,亟需加强公众的医学科普教育,提倡健康的生活方式,同时积极治疗血脂异常、避免超重和肥胖。ObjectiveTo investigate the prevalence and risk factors of hyperuricemia in rural adults in Hainan Province.MethodsA total of 1 120 people aged 18 years old and above in rural area of Hainan Province were selected by multistage stratified cluster random sampling method from 2018 to 2020, and the current situation and risk factors of hyperuricemia were analyzed based on such sample.Results23.6%, and the standardized prevalence was 31.9%. The crude prevalence of hyperuricemia in men was 50.1%, and the standardized prevalence was 57.6%. The prevalence of hyperuricemia in young men(18-<45 years old) was the highest, which was 53.5%. The crude prevalence of hyperuricemia in women was 9.4%, and the standardized prevalence was 9.1%. Among the hyperuricemia patients, 26.1% were associated with diabetes mellitus, 39.0% with hypertension, 55.3% with dyslipidemia,57.6% with overweight and obesity, 47.7% with metabolic syndrome, which were significantly higher than those without hyperuricemia. Logistic regression analysis showed that high BMI and LDL were the risk factors of hyperuricemia in rural adult men in Hainan province, and high FBG was the protective factor;High age, BMI and LDL were the risk factors of hyperuricemia in rural adult women, and high HDL was the protective factor. The prediction model of male uric acid was UA=351.443+3.867×BMI-9.322×FBG+3.197×2hPBG;The prediction model of female uric acid was UA=286.236+4.195×BMI-17.024×HbA1c+18.028 × hypertension + 2.966 × 2hPBG + 13.163 × dyslipidemia.Conclusions hyperuricemia in rural adults in Hainan Province was very serious, especially the young men. It was urgent to strengthen public medical science education, advocate a healthy lifestyle, actively treat dyslipidemia and avoid overweight and obesity.
