摘  要:目的 分析深圳口岸出入境人员与当地人群中流行的HIV-1 CRF01_AE毒株的基因相关性,为制定相应的防控政策提供指导依据。方法 以深圳口岸出入境人员与当地人群中HIV-1CRF01_AE毒株感染者为研究对象,获得两个人群中的HIV-1基因序列并判定基因型,分别应用传播网络和系统进化分析方法,研究两个人群中HIV-1CRF01_AE流行毒株的关联情况。结果 本研究获得2003—2014年途经深圳口岸的49例出入境人员HIV-1CRF01_AE感染者,以及111例2001—2014年深圳当地人员HIV-1 CRF01_AE感染者体内艾滋病病毒pol基因序列。深圳口岸出入境人员及当地人群中的HIV-1 CRF01_AE毒株传播网络模式图显示,6例中国大陆籍出境者感染的病毒与深圳当地流行毒株有较密切关系,2例香港籍、3例越南籍入境者体内毒株与当地流行毒株关系较密切。深圳口岸出入境人员及当地人群中的HIV-1 CRF01_AE近似最大似然法系统进化树显示,集中包含两个人群的HIV毒株基因序列所在的进化簇,并且可靠性检验系数在0.9以上的有五个。从系统进化树所处的位置,观察到1例马来西亚籍、1例韩国籍、3例香港籍的入境者以及4例中国大陆籍出境者体内毒株与深圳流行毒株相关。结论 深圳口岸出入境人员与当地人群中流行的HIV-1 CRF01_AE毒株之间可能存在关联,应持续加强对国境口岸出入境人员的HIV分子监测。Objective purpose of this study is to analyze its relatedness of circulating HIV-1 CRF01_AE strain in local population and Shenzhen port enter-exit personnel, so as to lay the foundation for the formulation of policies.Methods obtained from the entry-exit personnel and local people in Shenzhen, and the genotypes were determined. The transmission network and phylogenetic analysis were used to study the association of HIV-1 CRF01_AE in the two populations.ResultsIn this study, the pol gene sequences of 49 HIV-1 CRF01_AE infected entry-exit personnel at Shenzhen port from 2003 to 2014 and 111 HIV-1 CRF01_AE infected local population from 2001 to 2014 were obtained. The network pattern of HIV-1 CRF01_AE among entry-exit personnel at Shenzhen port and local population showed that the virus strains from six Chinese mainland exit personnel, two Chinese Hongkong immigrants and three Vietnamese immigrants had a close relationship with the local epidemic strains in Shenzhen. The phylogenetic tree of HIV-1 CRF01_AE among entry-exit personnel at Shenzhen port and local population showed that there were five evolutionary clusters containing the viral gene sequences from these two populations, and the reliability test coefficient of these clusters were more than 0.9. Based on the position of the phylogenetic tree, it was observed that a Malaysian immigrant, a Korean immigrant, three Chinese Hongkong immigrants, and four Chinese mainland exit personnel were associated with the local epidemic strains in Shenzhen.Conclusion association on circulating HIV-1 CRF01_AE strains the local population and the entry-exit personnel at Shenzhen port, and the HIV molecular surveillance on entry-exit personnel at border ports should be continuously strengthened.
